[Question #10033] Risk of HIV from receiving oral sex

Hello,

About a month ago, I recieved a blowjob from a stripper. I used a condom, but she put it on with her mouth. The whole thing lasted probably 20 seconds. I don’t know her STI status, but given her line of work I’m concerned. 

Online, and even in this message board, it says that the transmission is very low; but not zero. I would imagine that is reduced with use of condom. At the 3 week mark (day 21) I got a blood test and Gen 4 HIV test that came back negative. I was feeling good but when I spoke to The doctor they said I should rest eat again at the 45 day mark just to be conclusive because I was a little early. Should I consider the test on day 21 conclusive? I’ve had extreme anxiety and the thought of waiting another 20 days is killing me. And I also would like to continue with my sex life but am afraid of the chance of infecting others (if I caught it). Anything you can provide is greatly appreciated

Welcome to our Forum.  Thanks for your questions.  I'll be glad to comment.  The risk of getting HIV from the encounter you describe is virtually zero.  Most strippers/commercial sex workers do not have HIV and even when they do, even the most conservative sources such as the CDC estimate that fewer than 1 in 10,000 unprotected acts of fellatio with untreated, infected partners lead to HIV infection.  These estimates are conservative.  Despite being actively engaged in research and care on the topic, neither of us has ever seen or heard of a case in which receipt of unprotected oral sex led to HIV.  Further, in your case, a condom was used further reducing the already virtually non-existent risk for infection.  Were I you, I would have no concerns and would not feel the need for testing.  In your cases, your day 21 blood test provides further, although not technically conclusive evidence that you were not infected.  You are more likely to be struck by lightening today than to have gotten HIV from the exposure you describe.  While your doctor is technically correct, I see no reason for concern.  Please don't worry.  I am confident it is OK for you to resume regular, unprotected sex with your regular partner(s) without concern that you might be transmitted HIV acquired through this episode to them.

Thank you for this. This helped put my mind at ease a bit more. If I were to re-test, what is the earliest I can test that would give me a high degree of confidence to put all this behind me? Thank you

At 4 weeks (28 days) testing would detect more than 99% of recent infections.  At 6 weeks, results are 100% conclusive.  EWH---

Thank you! One more follow up - my doctor said that at 20-21 days that the test has “some accuracy”. The test was a HIV Ab/p24 Ag with Reflex. Do you know what percentage accuracy a test like that is? I was hoping for something a little more concrete (numbers wise) that what my doctor was able to provide. Again thank you.

At 20-21 days your HIV tests would have detected about 50% of recent HIV infections, perhaps slightly more but would not be completely conclusive.  At 4 weeks tests would detect of 99% of recent infections and by 6 weeks results would be 100% conclusive.  I'll repeat however that the encounter you describe above was a virtually no risk event.  Please don't worry.
