Hello doctors. I have a few follow up questions about my exposure and remaining symptoms.
As for HIV, its been almost 3 months since my “false positive” P24 Antigen test. For the last almost 2 weeks Ive been feeling very sick. Mild fever that comes and goes, headaches, joint and muscle pain. Is it possible that the P24 test was actually accurate and Im just a late seroconverter and Im experiencing ARS now or something alike? Im really afraid of testing again since that “false positive” result. For almost a year Ive been having GI issues with nausea, constipation, diahrrea, I just read online someone that also lasted a year with GI issues to later found out he was HIV positive.
1) Could this be a case of late seroconversion? Could I be positive if I test again?
As for other infections and Dr, Handsfield last comment, I did test for Gono and Chlamydia and other bacteria, I actually test 4 times, 2 urethral swabs and 2 urine tests. Everything negative. The reason I keep testing is because I keep having all these symptoms. My regular partner has also been complaining lately about urine frequency and pain with menstruation and sometimes during sex. Shes going to visit her GYN but Im afraid theyll find something I wasnt able to find with my tests (I could be 100% sure about her fidelity, so if theres something there it was from my one exposure I mentioned originally)
2) Do you believe this could be some false negative results?
3) After the exposure I spent 6 months before having sex with my partner again. During that time I took some antibiotics for other reasons. I took 1G of azythro 5 months post exposure. I took 400mg of Cefixime for 10 days like 2 months post exposure, I took augmentin for 10 days like 5 months post exposure. If there was indeed an infection, would any of this have cured it before I resume sex?

Just to be clear on the medicine I took:
2 months post exposure: 400mg of Cefixime, 1 dose per day, for 10 days
5 months post exposure: Augmenting for 10 days
5 months post exposure: 1G of Azythro
I know Chlamydia could be very “silent” and I know azythro doesnt always work against it so, could this be a possibility?
My tests for these infections were between all this antibiotics, could this be causing false negative results?

Welcome back, but I'm afraid this forum can't help much more than we already have.

1) As for HIV, the answer to "Could this be a case of late seroconversion? Could I be positive if I test again?" is obvious from your recently closed thread. Re-read it. The answer is no.
2) I'm glad to learn you were tested for other STDs. In fact you were over tested -- undoubtedly reflecting your emotional inability to believe the results. All those tests are highly reliable and almost never give false positive results.
3) There was no rational reason to wait that long before resuming sex with your partner; you could have safely done so as soon as confirmatory testing showed your initial HIV test result was false positive, and after your first round of negative tests for other STDs. You did not need those antibiotics, but the answer is yes: they would have cured you if somehow your gonorrhea or chlamydia tests were falsely negative. It is impossible that your partner's s symptoms are due to any STD from you. There are plenty of non-STD explanations for symptoms like hers. She should follow her doctor's advice; and you shouldn't worry about it.

Thanks doctor. I just want to clarify:
- I resumed sex 6 months after the exposure. At this time I had 2 urethral swabs with negative results and a few Ag/Ab HIV tests (because one doctor here told me it could take 6 months). Also had one HIV PCR test. After all this is when I resumed sex.
- I kept testing for HIV almost monthly and it was not until the 12 months mark that I received the positive P24 Antigen results.
Just to clarify I didnt get the positive results and confirmatory tests before resuming sex but 6 months after, 1 year after exposure.
1) I didnt take these antibiotics for STIs but for other issues, Cefixime after a surgical procedure, Azythro and Augmentin for tonsilitis. But since I took it for whatever reason I wanted to know if they could have cured anything. How could this medicine cured the infections if present? I understand for Gono the treatment if Ceftriaxione injected, I didnt have this. And for Chlamydia will be Doxycyline that I didnt took either. And most times the recommendation is to take some of this in combination and none of the antibiotics I took were at the same time.
2) You say the tests for Gono and Chalmydia almost never give false positive. Did you mean false negative?
3) How can I be sure I didnt have a false negative result on tests? With all these symtpoms I keep having like penis pain, urine frequency, testicles pain and my partner also complaining about genitourinary symptoms
4) Ive been to the doctor many times for all my symptoms. Nothing shows any reason, no test results no examen no imaging nothing, so I must assume something must be wrong isnt it? Something must be saying that theres nothing wrong and is mistaken. I assume is the STI tests because all of this so coincidentially started just a few days after I had that exposure 15 months ago. What are your thoughts?

Sorry I misunderstood the timing. Whatever it was, you were way seriously over tested. More comments below.

1) Glad to hear you weren't treated on account of your STD concerns. In any case, the azithromycin would have cured chlamydia if you had it, and the amoxicillin component of Augmentin also would probably have cured it. Augmentin isn't a recommended gonorrhea treatment but probably it would have worked.
2) Correct, false negative. Apologies for the typo.
3) False negative results are rare, and exceedingly unlikely to occur on several occasions. Your symptoms are NOT typical for gonorrhea, chlamydia, or any other STD. /they don't cause penile pain, urinary frequency, or testicular pain other than overt epididymitis, which you obviously did not have.
4) "I must assume something must be wrong...?" It depends on what you consider "wrong". It is clear that there is no infection or other physical reason for your symptoms. However, they are typical for the male chronic pelvic pain syndrome (CPPS). (You can google it -- spelled out -- for detailed information; the Wikipedia article is good. Also look for information from the Stanford University dept of Urology.) As you'll see, CPPS often has a psychological component, and your symptoms are completely typical for CPPS and genitally focused anxiety. Your serial testing and major over worry about all this also reflect your anxiety and stress, probably triggered at least in part by a sexual decision you came to regret. In other words, starting after the sexual exposure isn't a coincidence at all: that event is the direct cause. But primarily on the account of psychology, not an infection. You had a completely safe, condom protected exposure, for goodness' sake! Clearly your anxiety and shame over a sexual decision you regret is the main health problem here, in my opinion.

Hello doctor
I recently took another Ag/Ab HIV test and the result was non reactive. This test happened 3 months after I had the "false positive" P24 Antigen test.
My partner also had a Ag/Ab HIV test recently as part of her anual screening and it was non reactive.
I'm still worried about this results because I'm still feeling sick most of the time. Right now I've been having a constant headache for the past 5-6 days, couldn't this be related?
I've about HIV subgroups that cant be screened in a regular test like subgroup O and such, I've seen tests that specify when this group is being tests. My test didnt specify this.
Could my test be missing something?
Can I be 100% sure I dont have HIV with these results? No chance of missed diagnose or rare subtype or subgroup HIV or anything
I live in a third world country and I dont really know how good the tests are for this kind of things. Thats what worries me the most
This last test was roughly 15 months post exposure.

I'm sorry you are "feeling sick most of the time". As we have discussed, it isn't HIV or any other STD.
The online business about undetectable HIV types is an urban myth. And it applies only to HIV antibody, but you have been tested for HIV antigen and RNA as well. Those would be positive if you had type O or any other type of HIV. Your tests are not "missing something". All the HIV tests in developing countries are the same used anywhere else.
You do not have HIV. Ignore any other "what if" or "could I be the exception" sort of questions that may come to mind. Trust me on this: there is nothing you will think of that would change the fact that you are not infected.
That concludes this thread. The forum has a policy against repeated anxiety driven questions on the same topic. This being your second, it will have to be your last about your initial false positive HIV test and the other issues we have discussed. Such repeated question are subject to being deleted without reply and without refund of the posting fee.
