[Question #10168] Questions about syphilis testing

I have severe OCD and I know I upset you both when I ask questions, but I am really scared and can't get any straight answers, if you want to help me out and answer the question I will be very grateful, if you don't I understand and consider a donation to your fine organization.  I had two RPR tests in 2013 both negative - but my risk factors if they were risk factors were years before these RPR tests - anywhere from 3-11 years before.  I never had symptoms always used a condoms for vaginal sex, most of the time for oral but I did sleep with FSWs.  I also may have drank alcohol 24 hours before these tests, but can't remember.  Anyway my fear was that because most of the risk factors were years after I took the test I felt like I may have tertiary syphilis and the sensitivity for RPR test in that cohort is around 73%.  You guys assured me I was fine, although I never mentioned I may have drank 24 hours before.  Having OCD I decided to get the reverse algorithm done were I had an EIA / IA test last year, it was also negative - my questions (1) when I try to find sensitivity / specificity for this test its always NA for the tertiary cohort - why is that?  (2) did I get the wrong test done?  Or can you use sensitivity / specificity for the overall performance of all stages? (3) Am I negative or do I need to take a TPHA or FTA-Abs test to truly find out if I have tertiary syphilis because I was so far from "supposed" risk factors that would be the only thing I could have if I have syphilis?  (4) Is all this meaningless and I am syphilis free since I had no real symptoms - no chancre, never had sex with men, etc.  Please answer this - I am really suffering, and my OCD while improving still gets me from time to time - if you don't want to answer I understand.

1) Sensitivity and specificity don't matter. Negative is negative on all tests you have had.
2) RPR and EIA are the only appropriate tests and you had both, and...
3) ...you do not need and should not have any other syphilis test of any kind.
4) Yes, it's all meaningless and you are free of syphilis.

I'm truly sorry about and sensitive to your OCD, but this forum isn't the place to deal with it.
