Hi Drs, 

Thank you for your help to my last question. As per your suggestion I went ahead and got a panel test at exact day 21. I went through stdcheck.com and used their early detection hiv rna test which is supposed to be conclusive after day 20. I talked to their representative for a confirmation as well. This made me feel better as everything came negative which isn’t a surprise as I rarely participate in any risky behaviour and have always used protection. I wasn’t anxious any more. It is currently day 35 and I’ve had no symptoms which makes me pretty confident that I am risk free and didn’t feel the need to test again. 
Now the issue is I had protected vaginal sex with my partner and exactly on day 2 infact less than 48 hours she’s started having soar throat. 4 days now and she has severe soar throat, chills, nausea, headaches, body pain, cough a bit and a runny nose. This has got me concerned. I know this can be covid as well as symptoms are so close. Waiting on her to test for covid. The only issue is I have been close all the time and didn’t catch anything. So it makes me questions whether it is actually covid or I gave her something.

Hi Drs not sure if this question was missed. 
I do want to mention I strongly think this is a common flu 
As 
1) I tested negative using hiv rna test at day 21 
2) I personally have had no symptoms 
3) 2-4 days might be too soon for any symptoms to show up? 
4) Condom was used and there was no slippage or breakage

Hi apologize for the delay in responding. I attempted to answer yesterday and had an Internet glitch.

I understand how a flu-like illness in your partner following a potential exposure to another partner might be concerning. I can assure you, however, that you have not transmitting HIV to your partner. Your HIV PCR test results prove that you were not infected. In addition, you used a condom during your most recent sexual encounter. Finally, the onset of your partner’s symptoms just two days after your most recent sexual contact is far too quick to be HIV.

Hi Drs I wanted to use the same open question for a different question. 

I had an interaction with a Csw where she performed brief oral sex on me. It was unprotected and maybe only lasted 30 seconds after which I insisted to put on a condom and she did. 
No other activities were performed. 
I checked with her if she’s tested and she didn’t agree nor deny. I think there was a language barrier and it was dropped. 

My question is are there any stds I need to worry about. I understand my actions are lowest of low risky for me. As the oral sex was performed on me. 

I do wanna know if I need to get tested or take any medicine. It has been only 2 hours since the incident. 

I’m worried about all the std possible. 

I came in negative for every std when I tested couple of weeks ago

Also I do want to add I’m uncircumcised. Not sure if it changes anything. I’m mainly concerned because of that brief unprotected oral encounter

Amongst penetrative sexual encounters, receipt of oral sex is among the lowest risk for acquisition of STIs.  Any single unprotected exposure can be characterized as low risk as most CSWs do not have STIs and most single exposures do not result in infection.  I estimate (there are no good data) that fewer than 10% of exposures to a person with oral gonorrhea (far and away the most common oral STI) lead to infection and perhaps far lower. There are no data on how the duration of receipt of oral sex impacts likelihood of infection but common sense would suggest that infections are more likely to be transmitted with longer exposures so your 30 second exposure before the condom was put on is in your favor..  There is no risk of HIV from this exposure, no m3eaningflul risk of herpes or chlamydia and syphilis would be very, very unlikely..

As far as testing is concerned, at any time more than 3 days after your exposure you can get a urine test for gonorrhea and chlamydia and expect accurate results.  You do not need antibiotics- there is little chance that you were infected.

Hi Dr thank you so much for your response. I have realised these risky activities are something I can not handle and plan to stop them completely. 

I will go ahead and get tested for Gonorrhoea and chlamydia next week. 

You mentioned no exposure to HIV. I want to add I did ejaculate in those 30 seconds before a condom was put on and then she gave me oral again. Does that change anything? 

Also I saw online some places suggesting taking pep if oral sex involved blood. 
I think there’s no way for me to know as I never looked at her mouth. There was no kissing involved either. Only thing I know is that penis was covered in saliva and I did not see any blood on me.

At the same time I find lot of conflicting advices online about oral. 

Can you please confirm if I need to take any precautions.

With or without ejaculation, there has never been a case of HIV which is been proven to have been acquired from receipt of oral sex.  This is the case no matter whether the person performing oral sex has poor dental, hygiene, sores, or blood in their mouth – still no risk! The sources that suggest that post exposure prophylaxis (PEP) is appropriate for persons who have received oral sex are totally off base and represent just how much misinformation is present on the Internet.

I hope this information is helpful to you. Please don’t worry. You are not at risk for HIV from the counter you describe.
