[Question #10286] Hiv negative with multiple infections

Hi doctors thanks for the continued hard work you do. I'll dive right in.
So I had penis to mouth oral with a bi sex man whilst getting fingered I was led to believe its low risk but not zero so after few days I left it in the back of my mind unfortunately 4/5 weeks later sore throat and stomach issues lost weight so took a test 4th generation and all other stds all negative but more mouth sores then followed up but at 9 weeks negative at 11 weeks bloods feom doctors said I had low folic acid and stool sample said h.plyri positive which antibiotics have ran there course but continued issues still unresolved. My skin if I have an abration from work are just not healing right also strange. Blantis and recurring oral thrush and anal sores comes and goes too. My clinician was ready to kick. Me out the door by 14 weeks but due to the recurring opportunistic infections she's keeping my file open and saw me at 18 weeks still negative next and last to cover 6 months period i have to wait until 24 weeks now as she's confused 30 years and never saw anyone past 90 days and 6 weeks using 4th generation turn positive.. My questions are
1. Is the risk great enough to have transmitted hiv?
2. Are the symptoms more like the virus has damaged the immune system rather than antibodies being present or antigen? Could this be causing a late positive would antigen still be picked up?
4. My wife has also developed terrible night sweats and mouth sores and sore throat is this all coincidence? She has no understanding why she's feeling so bad says she feels constantly sweaty and sick.
So out of sheer anxiety over all these symptoms I've booked a private clinic to run an hiv pcr test to see if they can detect the virus itself. This is now 20 weeks would that test be appropriate considered 2 weeks ago 4th generation picked up nothing.?

Welcome to the Forum. Thanks for your confidence in our service. I'll be glad to comment. The short answer is that you have completely ruled out any possibility that you acquired HIV from the exposure you describe. It's time to look for other reasons. To expand-
1. Is the risk great enough to have transmitted hiv?
The estimated risk of performing oral sex on an untreated, HIV infected person is less than 1 infection per 10,000 acts of oral sex and probably substantially lower. The cases are few and far between.
2. Are the symptoms more like the virus has damaged the immune system rather than antibodies being present or antigen? Could this be causing a late positive would antigen still be picked up?
Persons who acquire HIV do not have meaningful declines in immunity until years after acquisition of infection because it takes that long for the immune system to be damaged by the virus. The idea's of "late" or "delayed" positives are a myth which has been perpetuated by internet misinformation.

The PCR test is overkill and will only confirm the testing you have already had. You do not have HIV and need to look for other causes of the problems that both you and your wife are experiencing. They are most likely due to other, independent problems.
Further comments:
The antibiotics which cure H. pylori also have many other side effects. Check with your doctor to see if they could be part of the problems
Your wife's symptoms have many causes. In fact, night sweats can be caused by H. pylori and it is possible that you are both infected.
I am confident this is not HIV. Time to look for other reasons. If there are further questions, please use your up to two follow-ups for clarification. EWH

Thanks for your swift reply and reasoning words I am hopeful that I get a negative result today or at the latest Monday morning. Anyway its been very confusing how the timing of all these opportunistic infections have occurred I understand that if it was symptoms the likelihood my 4th generation would light up like a fire engine if it was hiv but like I've said I'm saying symptoms as these seem more related to my body's immune system not functioning properly hence why I have so many of these opportunistic infections so without hopefully sounding repetitive
1. Let's say my body hadn't produced enough detectable antibodies upto 18 weeks of 4th generation testing to be picked up can the virus itself cause these problems due to failure of immune response? And if this was the case would my p24 antigen for hiv-1 have a much more likely chance to be positive even upto 18 week's?
2. I believe my h.pylori has been effectively treated by the 2 week's course of antibiotics but is h.pylori something that's easily transmitted to others? as when asking my doctor about having my wife being tested she was rather dismissive.
I'm currently seeking other root causes for my health issues as they have very much came out of nowhere hence why the frequency of testing but I am very much working with my doctor to pursue other causes but so far unsuccessful at getting to the bottom of all this.

Just to update my hiv pcr test came back negative today which is very reassuring the only downside to this was it says on the report it's negative for hiv-1 only and not hiv-2. I thought it would have tested for both strains of the virus.

Straight to you from the questions:

1. If you had HIV a fourth generation combination, HIV antibodies/antigen test, which is the standard test used this time would certainly light up. Negative tests are conclusive.
2. No, H. Pylori is not an easily transmitted infection, and is certainly not sexually transmitted. However, you and your wife share meals and likely eating and drinking implements. H. pylori does have a tendency to occur within families.
I must say, I am not surprised at your PCR results, which absolutely confirm your earlier test results and proves without a doubt that you do not have HIV. Presuming you are here in the United States. I would consider your results conclusive. The fourth generation tests you have had earlier did test for HIV-2 and were negative. Furthermore, there are fewer than 50 cases of HIV-2 which occur in the United States each year and these infections occur exclusively in persons from West Africa or India or those who have had sex with them.
It is time to focus your energy, so I’m looking for other causes of your recent illnesses. I hope my comments will help you to do so. EWH.

Thanks doctor for all your answers and ofcourse it's certainly helped oh and I'm from the UK not the United States. I hope that doesn't make much difference in your opinion? Anyway I appreciate the help and have no futher questions to ask and I thank you again for all your wonderful knowledge on these matters, I will now go and enjoy my weekend and not burden my thoughts with these irrational fears.
