[Question #10287] Mycoplasma genitalium, Trich and Oral sex

I recently posted regarding a regretful incident where I recieved short unprotected oral sex from a massage lady in Vietnam. At around 12 days after the exposure I had urine tests for ghonarhea and chlamydia which were clear as I was suffering some.discomfort in my urethra and testicle, I also had the normal HIV etc even though not really a risk and was also clear. Event though I am confident in the results my symptoms did not dissipate, as I was travelling I visit a local doctor and they prescribed chefalexin for 7 days for a uti. The symptoms did reduce, I also has started to notice a wet feeling on the end of my uncircumcised penis but it was not heavy did not stain my pants and was clear not obvious except for moisture and wet sensation. I still had some discomfort as well. I then on My return home visited my normal doctor they sent me for a urine test for uti after the antibiotics had finished, again clear and they also put me on doxy 2 X per day for 14 days.
I am now on day 6 most of the symptoms have subsided but I still feel the need to urinate more than before but not constantly and some.sensation in the head of my penis, some moisture as above which comes and goes.
Should I be concerned with mycoplasma genitalium and trich as there is much confusing information regaring risk with only oral sex.
Also should I be concerned with infecting my long term partner if we return to unprotected intercourse which I have avoided until now.
Also I am now close to 30 day post the incident

Welcome back, but sorry you found it necessary -- especially in view of this very simple issue.
Trichomonas vaginalis never infects the oral cavity and thus not transmitted by oral sex. The data are not as clear for Mycoplasma genitalium, but most studies and the general consensus also is that it rarely if ever inhabits the mouth or throat and also is not acquired by oral sex. In addition, treatment is not recommended anyway for M. genitalium if there are no symptoms -- and therefore testing for it is not advised except in special situations nothing like yours. So you do not need to be tested for either of these infections.
Frequent urination and sensations at the head of the penis are not symptoms of any STI. Almost certainly your anxiety over these issues is making you more aware of trivial symptoms or even normal body sensations that otherwise would not be bothersome and perhaps not even noticed.
Ignore testimonials on non-professional websites by people who are "sure" how they were infected (like Reddit, for example). They often are wrong, and many are intentionally misleading.
The event you are concerned about was very low risk for STI to begin with. You should be continuing sex with your regular partner without worry.

Thankyou for your prompt response,
One further question with regards to the doxy I am on now if in case I do have a NGU type infection, is the 14 day schedule appropriate (2 doses of 100mg per day for 14 days)
Secondly given my primary symptom is a regular but not constant cold wet like sensation in my urethra/penis gland (a menthole type feeling i guess is best way to explain), Will symptoms normally subside by this time 6bl days into the treatment, and if they don't how long after the treatment should I wait for further testing if any.

You should not be taking doxycycline in this situation. But if you continue, the standard treatment is for 7 days, not 14.

The “primary symptom” you report does not fit with NGU or any STD. As you say yourself, this sounds psychological. IF it were due to NGU, the symptoms would have been gone within 3 days of starting doxycycline. By taking doxycycline, you have ruined any chance of accurate testing for the causes of NGU and maybe for gonorrhea. You’ll probably never know for sure whether or not you were infected or, if so, with what.

Hi Dr
Thankyou for your response, I have now finished the Doxy as prescribed by my local Dr. As tjis is my last question and I still have some discomfort in my testicles and penis from time to time, what would be the best timing to retest for an infection.

As already discussed, such on-and-off testicular or penile pain never are signs of infection, but they are typical for genitally focused anxiety. Failure to improve on doxycycline is not evidence of a resistant infection, but of a non-infectious cause. Further evaluation or testing are unnecessary and would be a waste of money, time and energy.
Trust me on this: you have no infection from the oral sex event on your mind. If you still doubt that, please go back and carefully read both your threads in detail. The answers are there.
