[Question #10306] Protected sex with csw, little sore on testicle skin

Hello doctors

I had sex with a CSW in my country today. She gave me unprotected oral for 1-2 mins and then protected vaginal sex. I just remembered i have a small itch (probably a little wound due to scratching) on the below part of testicle. Then i asked her if she gets any testing of std. She told me she regularly check for stds once a month, including blood test and vaginal exam by doctors. My question is :

1. Is my risk of std still big? Knowing i have unprotecred oral & wound on testicle?

2.She admit to me she did ejaculate. Is it possible to increase the risk of stds if the fluid touched the wound?

3. Should i do some medication to prevent stds?

Welcome to our Forum. Thanks for your confidence in out service.  I'll be glad to comment.  Before I ask your questions, let me assure you that symptoms of an STI virtually never begin within 24 hours of an exposure to an infected person.  Thus the itch you are experiencing in virtually certainly not due to an STI although it could be due to a non-infectious cause such as rubbing (friction) during sex of, if you or she used any sort of irritation, chemical, non-infectious irritation,.  Both of these possible causes should resolve quickly without any therapy.  In response to your specific questions:

1. Is my risk of std still big? Knowing i have unprotecred oral & wound on testicle?
The only STI which occurs with any regularity from oral sex is gonorrhea and that is uncommon.  Statistically it is unlikely that your partner had oral gonorrhea and the wound on your testicle in no way changes that possibility.  There is almost no chance of chlamydia and there has never been a case of HIV proven to have been acquired from receiving unprotected oral sex.  Your genital exposure was safe sex as long as the condom was worn throughout the encounter.  

2.She admit to me she did ejaculate. Is it possible to increase the risk of stds if the fluid touched the wound?
No this does not change this- it was a very, very low risk encounter.

3. Should i do some medication to prevent stds?
No, there is no need for any medication.  In fact, medication is more likely to confuse things than to offer any meaningful help

Thankyou for the reply dr. I need to clarify that the wound is because of my scratching due to itch before my intercourse (1-2 days prior). Hence :

1. What is the possible std if the csw’s ejaculation fluid contacts the wound?

2. Should i do any testing afterwards?

3. I feel bad because i have a regular sex partner. Should i stop having sex with her for some weeks until i do some testing?

I understand the nature of your wound. There is no change my assessment. Yes the answers to your questions are:

1.  None. STI‘s, including HIV are not transmitted in saliva.
2.  See my comments above. The only STI of any concern whatsoever is gonorrhea and that is typically symptomatic. If you wish to test however, that is the STI, I would test for.
3.  See my comments about testing above. I see no reason to abstain from unprotected sex with your regular partner.
