[Question #10308] Syphillis

Hi Dr, I have a question regarding efficacy of doxycycline mono 100mg in regards to Syphillis treatment. Is 14 days of this medication alone enough to clear earlier stages of the infection? Or does tetracycline have to be prescribed as well?

Welcome back. Thank you for a succinct question.

Doxycycline 100 mg twice daily for 14 days is 100% effective against syphilis under a year in duration, assuming certain complications (such as neurosyphilis) are not present -- in which case 4 weeks treatment is advised. The treatment of choice remains a single injection of long-acting penicillin (benzathine penicillin G [BPG], trade name Bicillin LA®). However, there is a national shortage of BPG, and therefore doxycycline for 14 days is advised by CDC for those persons with early syphilis who are likely to remember to take all doses on schedule.

Tetracycline probably would be as effective but is not recommended, primarily because it must be taken 4 times daily and also more likely to cause stomach upset, therefore more difficult for some persons to take successfully.

Okay that makes complete sense. My follow up question would be is there a possibility that 10 days of doxy taken as advised would clear an early infection? Thank you so much for your prompt response.

In exposed persons in whom overt syphilis has not yet developed, 7 days of doxy is effective in preventing syphilis from taking hold. Once symptoms start or several weeks have passed, 10 days probably would be sufficient for cure but has not been studied, so it isn't possible to know with certainty.

If you were treated with doxy to prevent syphilis -- i.e. after exposure but before primary syphilis developed -- 10 days is plenty.
