[Question #10314] Oral sex and HIV Worry

I'm a young gay live in Asian Country.

Yesterday, I go to Sex worker shop. We have mutual masturbation he lick on my body (which I think it's not a risk for HIV .)

Then he perform a Oral Sex on me (His mouth on My penis) for a while until I ejaculate in his mouth.
that's put me to worry as He oral to me for a long time until I ejaculate. I didn't perform a oral sex for him. I quite worry about this.

Would this pose me a risk for HIV ? I do not worry about other STD as it can be cure.

Another question, As I'm a very anxiety person. So the doctor give me a Prep on Demand which should take at least 2 Hr before activity. and I didn't take that 2 Pills before receive oral sex as described above but I take 2 pills 1 hr after finished the activity.

my question is 

1) delay of taking 2 pills would make Prep not efficient as It should be, but do it have any negative effect?

2) Should I continue the other Prep which will take another pill on next 24 and 48 hrs ?

Oral sex is very low risk for HIV. The US Centers for Disease Control and Prevention (CDC) has published an estimate of one chance in 20,000 of catching HIV by receiving oral sex, i.e. for the penile partner. And that's only if the oral partner is known to have HIV. This figure is equivalent to receiving oral sex by HIV infected partners once a day for 55 years before becoming infected. (For the oral partner, if the penile partner has HIV, the risk is once for every 10,000 events -- equivalent to daily exposure for 27 years before infection would be likely.)

Because of those low risks, most experts would not recommend either pre- or post-exposure prophylaxis (PrEP, PEP) in persons only having oral sex.

As you understand, PrEP on demand is one approach to HIV prevention, but is not as effective as continuous PrEP; in fact, it takes up to a week or even two weeks of continuous PrEP to reach nearly 100% protection against HIV. However, you are asking about taking your PrEP drugs as PEP. Taking it 2 hours after exposure certainly is not harmful, and it probably worked just like PEP normally does:  taken within 3 days after exposure is nearly 100% protective. I see no need to take any further doses of drug at this time. As already discussed, even without PrEP or PEP, your chance of getting HIV from this one event is no higher than one chance in 20,000 -- and even that is only if your partner has HIV.

Thanks for your reply.

1) I have read on this forum, Receiving of Oral Sex (Blow Job) is less risk compare to giving right? and there are no proven case of transmit HIV from receive of oral sex (mouth to penis) but there have a few (rare) of transmit HIV from giving oral sex (penis to mouth)? SO I should not worry about HIV from receiving oral sex at all even I ejaculate in his mouth ?

2) Suppose I take only 2 pills of Prep but don't take further dose as you suggest, hence it will not complete the 2-1-1 formula. This will not have any negative effect eg. Drug resistant (as I alway told by doctor not to skip any Antibiotic) ?

3) and suppose I decide to take Prep as a PEP, how I take it? 2-1-1 or 2 then 1 pill per day for another 28 days.

4) base on my exposure (Receive oral sex). you advise that I don't need any PEP or Prep right? and how about HIV testing relating to this exposure ?

1) Yes, all correct.

2) I'm not telling you how to take the drug; best to follow your doctor's advice. 

3,4) Why are you taking PEP at all? There is no risk of HIV after receiving oral sex.
