[Question #10345] Persistent Symptoms Still

Hello, Drs., I've tried to accept your advice unfortunately I am still at a loss and have been unable to ignore ongoing and worsening symptoms. Mostly with my skin. I have ongoing dyshidrotic eczema on my fingers with no history of eczema. They are flesh colored bumps that appear randomly on the sides of the fingers, ongoing for over six months. Recently however I have been having more skin/rash problems. I have small pen point size red dots that mostly appear on the underside of my forearms and upper arms. They are smaller and brighter red/pink/purple than acne. Recently I've also developed these on my face near the eyebrows and hairline. Some are on my scalp too, they come and go and are slightly painful when touched. About two weeks ago I developed red spots on my nose that came and went and are back. I have a bright pink pea sized lesion on my left stomach that is almost completely flat. I have some peeling on my feet and some red spots there, including one bright red flat irregularly shaped one that came and went. Last week I also had a rash in the center of my right palm, one spot became three and went away after 3 days. I have bright pimples on my back as well. I also have intermittent joint pain, mostly in my left toes but sometimes in the other foot and fingers. I've had 14 negative tests since my last sexual encounter which was 5/1/22. Most were GenIV but some RNA. Most recent in early Aug. I know you're not going to recommend further testing, but I am at a loss. I'm not good with the medical system and I don't know whether to go to a dermatologist or infectious disease specialist. I am unsure if I should request qualitative or CD4 tests. I feel OK physically aside from the slight joint pain, but honestly all the bumps and lesions are starting to make me look sick and contagious in public, quite a problem. Could any of this be lingering PEP complications? Symptoms started when I was on it. Any advice on how to proceed would be much appreciated.

Welcome back to the Forum although I'm sorry you felt the need. In preparing to address your current concerns I reviewed your prior interaction with Dr. Handsfield and agree with all that he said. Early on in your exchange with him he replies- "your negative test results prove both that you do not have HIV (assuming no more recent exposures not mentioned) and that HIV is not a possible cause of all these symptoms -- no matter how persistent they are or how "weird" they seem. There is no point in further RNA testing (which doesn't detect any more infections than the antigen and antibody tests), or any other HIV testing of any kind." that remains the case and has been proven repeatedly with your 14 negative tests.

My advice to you is:
1. Stop testing. You do not have HIV and need to accept this. Further testing is a waste of time and resources.
2. See a dermatologist about your rash and skin problems. Anxiety in and of itself can lead to dermatologic illness and can amplify existing dermatologic problems
3. There is no realistic way that these new dermatologic issues could be due to the PEP that you took for your very, very low risk exposure that seem to have set all of this in motion.
