[Question #10357] Unprotected oral with mild symptoms

Hi Doctor,
I stupidly went to a bar in South East Asia, kissed a few random girls, and then had unprotected oral bj for like 30 seconds (mainly handjob). Lots of regret about that.
- 7 days later my penis head is slightly red and testicles feel uncomfortable, but no sores. Also I think I had some mild fever and muscle aches.
- 9 days after exposure I have sore lips and an ulcer like sore appear inside the lip/mouth.
- since then I feel ok, but very mild fever seems to come and go.
- got tested for HIV 4th generation, syphilis, Ghonerea and Chlamydia 19 days after, but all negative. Also doctor didn't seem too concerned.
Now it's been 5.5 weeks after, and I still have somewhat tender balls, and fever/fatigue seems to come and go even so many weeks later. Lip sore seems to finally be going away after like 4 weeks.
Any thoughts of what is happening, and do you think I'm at risk for HIV and should be retested? Thankyou

Welcome. Thank you for your question.
Oral sex is safe sex: not completely free of STD risk, but much lower chance of infection than unprotected vaginal or anal sex -- and zero risk for some of them. In the 40 years of the worldwide HIV/AIDS epidemic , there have been very few HIV infections (if any) transmitted mouth to penis. The handjob was definitely zero risk for any STD. Probably more important, though, your negative test results are conclusive for gonorrhea and chlamydia, which are the only STDs that possibly could have caused the symptoms you describe. Your negative HIV test is pretty reliable; conclusive results with the AgAb (4th generation) blood tests are not until 6 weeks, but since the exposure was zero risk for HIV, it really doesn't matter. Having not had a chancre (the open sore of primary syphilis), you can be very sure you didn't catch syphilis -- but the blood test isn't conclusive until about 6 weeks.
As for your symptoms, they really aren't a major concern. Symptoms that come and go like you describe almost never indicate any infection, and there's no STD that would cause any of the symptoms you describe. I suspect some (most? all?) are just anxiety making you more aware of minor symptoms or even normal body sensations that otherwise wouldn't be bothersome and you might not even notice.
Honestly, I don't think you need any further testing at all. For sure there's no point in another test for gonorrhea or chlamydia. Even though HIV is impossible and syphilis exceedingly unlikely, feel free to test again for both at 6 weeks if the negative results will give you more confidence about the situation.
