[Question #10449] Recent massage oral exposure

Hi Docs,

I need some urgent help. I went to a full body sensual masseuse earlier today. She explained all the rules to me, including no intercourse and only oral contact. She started by giving me a lot of licking all over my back when I was face down. Ultimately, she started to lick my butthole (aka rimming) which I could not stop her from doing. She also sucked my scortum and gave me a hj while doing all this. This happened for 10 mins, and I was able to cum. I got cleaned up, dressed and left her.

I am now scared for different forms of STIs, including HIV as I read it is low risk but not zero risk. Also, I had tested a couple of months back and was clean. This is the first encounter after that.

1. I don’t want to ask this question again, as it has been asked and answered several times but is this low risk or no risk activity?

2. Can I start on Prep or some other form of medication to prevent myself from STIs? 

3. Even if your recommendation is no, and I do want to take medication on precaution - where in US can I find the medication?

3. Do you recommend I do anything over the next week or so, to assess my situation?

I realize I posted this in a wrong forum but my real worry is now becoming about HIV prevention and disease. I would like to know other answers but also about my need to do PEP before the window closes (even if it is 0.005% chance). Please let me know.

Welcome to the Forum.  thanks for your questions.  I'll be glad to comment.  The interaction you describe was virtually no risk.  It really does not warrant concern of any sort, there is no need for testing and I certainly strongly recommend against PEP in this situation.  In reply to your specific questions:

Asking the question again will not change the answer.  This was a no risk event.  Rimming or her oral contact with your scrotum did not put you at risk for any STI, including HIV

2. Can I start on Prep or some other form of medication to prevent myself from STIs? 
You do not need preventative medication for this exposure.  You need to put your unfounded concerns aside.

3. Even if your recommendation is no, and I do want to take medication on precaution - where in US can I find the medication?
I do recommend against it.  I suspect you might find someone willing to prescribe medication that you do not need but I would not know whom that might be.

3. Do you recommend I do anything over the next week or so, to assess my situation?
I would not be concerned.

There is zero risk for HIV from the interaction you describe.  EWH

Thank you Dr Hook. This is indeed very helpful. The masseuse I was with, she told me she is very protective and clean. This had made me get a little more loose than what I typically do in such encounters which is handjob.

I have a few follow-up questions:

1. Reading about the risks of STIs (not HIV), I have scheduled to pick-up Doxy-PEP. Will that do any harm?

2. In future encounters, is it safe to just get nipple sucked and armpit licked? Do these kind of oral interaction carry any STI risk?

3. Do I need to take any precautions in terms of sex with my wife following this encounter?

It's frustrating to us when clients ask for our opinion then choose to ignore our science based advice.  That said, here are my responses to your questions:

For you to take Doxy-PEP is a waste of time and resources.  As long as you do not have side effects however, there should be little risk 

2. In future encounters, is it safe to just get nipple sucked and armpit licked? Do these kind of oral interaction carry any STI risk?
There is no risk from having you nipple sucked or armpit licked.  No one has EVER been infected with HIV or other STIs in this way.

3. Do I need to take any precautions in terms of sex with my wife following this encounter?
No!  Her pregnancy makes no difference in my advice

Hi Dr. Hansfield,

I will like to get your opinion to my situation. I see that you responded back to question #10455, that there is little risk of rimming/analingus. My window closes soon and I don’t know if the masseuse had a cut or blood drops on her tongue. Should I start on HIV PEP treatment? I understand that the risk is very low, but I would still like to consider that given I have a big family responsibility and cannot take the risk of HIV infection. I would really appreciate your suggestion. As mentioned, please consider if there was any cut on her tongue or blood drops (none noticed) and when she was rimming me. I am willing to take 30 day therapy for PEP.

Also, if I do start the therapy, are there any risks? Do I need to worry about any side effects or abstain from sex for the next 30 days. Awaiting your response.

For example, I have found the below article which states there is risk of transmission. I don’t mean to challenge or offend but just want to confirm and be sure.

As you know, we provide up to 3 responses to each client's questions.  Thus this will be the final response as part of this thread.  Further, Dr. Handsfield will not be answering.  He and I share the Forum and in the nearly 40 years we have worked together, while out verbal styles may differ, we have NEVER differed in the substance and facts of our responses to clients.  Clients are not permitted to request answers from a specific moderator.  

You have misinterpreted Dr. Handsfield's response to 10455- if you read the response more carefully you will se he summarized his response by saying there was no risk and no need for testing of any sort.  You have also been misled by the on-line article that you found.  You will notice that this is not a scientific paper and contains no references. Our responses are based on scientific papers and studies, attendance at scientific meetings with other specialists and our own combined experience.  We have never seen nor heard of a case of HIV acquired through rimming- you are not going to be the 1rst.  Whether or not she had blood in her mouth or tongue curs makes no difference.

As I said before this was an absolutely NO RISK encounter with no need for testing and certainly no medical or scientific need for PEP.

My final advice is for you to stay off the internet- much of what is there is incorrect, either because it is out of date, taken out of context or just plain wrong like the article you found. The internet is not your friend here.
