During massage in US with an asian woman, she grinded/humped my penis.  She was wearing underwear and I’m 90% sure I was covered with towel, never exposed.  10 days later on Fri, I began experiencing pain in groin/bladder, swelling in both testicles. I had some doxy and took 1 tablet on Saturday, went to urgent care on Sunday as everything got worse and was prescribed 3days of Levaquin until I could see a urologist.  I wasn’t experiencing discomfort peeing at all at this time.  Took std panel that Monday, 12 days after encounter and all negative - by this time all I took was 1 doxy tablet and 1 250mg Levaquin tablet. Could this have impacted the NAAT urine test? Urinary frequency increased and I began experiencing intermittent pain peeing and itching in penis. Took another NAAT test for Chlamydia and Gonorrhea at 21 days and all negative (urine sample was watered down though). By this time all I took was the 3 days of Levaquin.  Urinalysis/culture at clinic is clean. Convinced I have Chlyamydia. Had prostatitis in past 2-3x. I’m 51.  Could antibiotics have influenced any of the std testing? Are the NAAT urine tests accurate? - think I drank a bottle of water before one and gave to much on another. Could this just be BPH/Prostatitis or another non STD bacteria? Worried I could infect wife.  Now 7 weeks after.

Welcome to our forum. Thanks for your questions. Let me start by informing you that the encounter you describe did not put you at risk for any sort of STI. Grinding which is also called frottage is a a no risk activity. In addition, no STI so transmitted through underwear or other material, even if that material gets wet with genital secretions. This was a no risk encounter. 

Taking antibiotics before testing is unwise and can negatively affect tests in unpredictable ways. In your case however, you should believe your test results, since the encounter was no risk.

Other comments:
1.  Urinary frequency is not a sign of any STI although it can occur with urinary tract infections or prostatitis.
2.  Your symptoms are far more compatible with a recurrence of your prostate problems than any STI. 
3.  Finally, I would not worry about transmitting any STI to your wife.

Humor me - if by some chance her exposed vagina rubbed my exposed penis head wouldn't it have to rub up on the urethra versus the shaft for anything like Chlymdia to transmit? The 2nd NAAT test I took, although watered down, was 21 days after taking levaquin 250mg for 3 days.  If I was still raging with urinary discomfort and swollen testicles wouldn't the test still detect the bacteria if present?

No matter what the site of frottage, without penetration there is no risk of an STI.  If you were infected your test would have been positive.  It’s time to put your concerns aside.  EWH ---

Dr, are swollen testicles, scrotal discomfort/pain, intermittent pain urinating, increased frequency at times all symptoms of BPH/Prostatitis?  Its crazy that this all started 10 days after this event. I took 1000 mg of Azithromycin and then 500 for 3 days and I still have these things happening to a lesser extent.  I had an pelvic MRI done last week as a followup from a cyst they saw a year ago and it showed a mildly enlarged prostate measuring 4.9 cm. Would you think antibiotics are the preferred method to treat prostatitis?

BPH would not typically cause these symptoms but prostatitis could.  Clearly these problems are not associated with an STI from the encounter you describe.  My advice would be to see a urologist to help with your assessment and management of these symptoms.
