Dear dr thanks for your efforts
I went to csw 4 days ago was african female and its not expensive im sorry to say that its very cheap .. we kiss deep but not alot she do handjob for me untill my penis be hard i suck her nipple for just 3 seconds and she put condom on my penis and start to suck untill i cum inside the condom it was intact cause all my semen keep inside not gettin out then i remove with tissue then i left .. i have herpes oral under my lips and have small mouth sore as well inside my mouth ..
1-Whats is my risk for getting HIV from this act .. i heard that kissing little risk not zero so i my anxiety drivin me crazy .
2-Do i need to test HiV or sti c
Can you give my your assessment in this please .. i still not develop any symptoms yet except my herpes is active btw before the act ..
Did my herpes double the risk to catch hiv from kissing and my mouth sore as well what is the percent of this
I didnt touch my wife after this event if you were me what will you do
Im really afraid from the kissin cause of my mouth sore and activation of my herpes during the act

Congratulations on your continued practice of safe sex. Deep kissing with exchange of saliva, receipt of masturbation, sucking a partner's nipple and receipt of condom protected oral sex are safe sexual activity with virtually no risk for acquisition of STIs, including HIV. I really see no medical or scientific reason for concern or for testing. In response to your specific questions:

I see no reason for any STI testing related to the activities that you describe

Can you please clarify my herpes and mouth sore is it have any effect in your advice .. it was active during the deep kiss .. my oral herpes down my lips not in the lips it self and one mouth sore inside my mouth did it have any effect

The presence of an oral herpes lesion or inside your mouth does not change my advice in any way. Still a no risk event. EWH---

Hi Dr Hook hope you are fine .. I'm sorry to come back after you respond makes me relief but i starts on my day 13 after encounter started sore throat and muscle pain especially in my neck and shoulder and up of my kneed and still ongoing till now .. there is no fever at all or rash or enlarged lymph nodes .
I got fear for maybe ars or something what's your idea regarding this .. cause i started already unprotected sex with my wife .. thanks dr

Welcome back. I’m sorry to hear about your sore throat and flu like symptoms. I understand that these are some of the symptoms of the ARS but the likelihood that they represent HIV from the encounter you described is virtually zero. This is most likely another non-STI viral illness such as influenza, COVID, or another community acquired illness. You can prove this by getting a 4th generation, combination HIV antigen/antibody test. If your symptoms were due to HIV the test will be positive. I’m confident that the test will be negative and reassuring.

Thanks dr hook for your response i made today exact 21 days after my encounter 4th generation test which came back negative .. should i retest in 6 weeks .. can i certain know that my symptoms nos is not due hiv .. do u have any advice for me

Three weeks is a bit early to test if you had been exposed to HIV. As I told you however, the activities you described did notput you at risk and there was never a reason to test. A negative test will prove that your symptoms were not due to HIV.
FYI, testing for HIV with 4th generation, combination HIV antigen/antibody tests will detect more than 99% of recent HIV infections at 4 weeks and will be 100% conclusive 6 weeks after an exposure.
