CSW gave me a protected blowjob. Before bj, she kissed my nipple, then she sat on my belly for 5 seconds, then started bj. I’m not sure if her vagina accidentally touched my penis when she sat on my belly, but I’m 100% sure my penis didn’t penetrate her vagina. So this is what I’m worrying about. I got 4th gen test on 31th, 55th and 92th days. I asked Chinese doctors. Most Chinese doctors said 100% conclusively after 90 days.  A small group told me 6 months. My question:1. What’s my risk in statistical probability of getting HIV. 2. Chinese occupational exposure guideline wrote “after occupational exposure, people should use PEP, and have follow-up antibody test at 2, 4, 8, 12 and 24 weeks.” Why 6 months? US also 6 months several years ago. Any scientific reasons?  3. Why some Chinese doctors said 6 months even for 4th gen?

Addition to Q1. I especially worried about my penis accidentally touched her vagina for 5 seconds.
Q4. In China, you cannot donate your blood if you have suspicions unprotected sex within 1 year. That’s the rule. Why? Is that because there are some other disease have longer window period ? Or HIV window period is 1 year?
Q5. Do I need further tests? Can I have a baby with my wife now without worry?

Addition to Q3. I asked those doctors , they cannot say why 6 months. They said they didn’t see anyone turn positive after 3 months, but they insisted to say 6 months. That’s so weird and scary!

Welcome to our Forum.  Thanks for your questions.  Dr. Handsfield and I share the Forum and on this occasion I will be answering your questions.  Without penetration the exposure you describe was a no risk exposure for HIV or other STIs.  Thus if you had asked about the need for testing, I would have said it was not necessary.  Having tests, if you were tested with the combination HIV antigen/antibody tests which are widely used at this time, reulsts would be conclusive at any time more than 6 weeks (42-45 days) after exposure or, if you took PEP, after completion of pep.  If you were tested with an antibody only test, the results of HIV antibody only tests are entirely conclusive at all times more than 8 weeks following exposure or, for persons who take PEP, 8 weeks after completion of PEP.  Thus, in response to your questions:

1. What’s my risk in statistical probability of getting HIV. 
Zero.

2. Chinese occupational exposure guideline wrote “after occupational exposure, people should use PEP, and have follow-up antibody test at 2, 4, 8, 12 and 24 weeks.” Why 6 months? US also 6 months several years ago. Any scientific reasons? 
Over time, the accuracy of tests, as well as the time it takes to provide conclusive results have improved. I mention the current recommendations above.  

 3. Why some Chinese doctors said 6 months even for 4th gen?
These doctors may be overly concerned or their information may be out of date.

Dear Dr. Hook,

1. Why no penetration = no risk? I wonder if she has a little vaginal fluid on her vagina, and touched my penis, will that be a risk for HIV? Still 0 risk?
2. For Chinese occupational exposure guideline, they are still use 24 weeks follow-up test as a rule, not the old guideline. Why do they do that? Is that because someone may be reactive on 4th gen test but unsure on WB confirmatory test? So the guideline asked 24 weeks? This guideline bothered me a long time. Lots of Chinese doctors cannot explain clearly to me.

1.  This is a fact, proven by many careful scientific studies.  Simple contact with infected secretions does not transmit HIV.

2.  This is a very conservative guideline which does not agree with the scientific literature on the subject.  It also does not agree with US CDC or WHO Guidelines.  I cannot tell you why this is the Chinese guideline.  It sounds like other Chinese doctors cannot tell you why either.

3.  I cannot explain this rule.  This is not the rule in the US where all blood donated for transfusion is tested for HIV as well as other pathogens before it is transfused.  This is well beyond the window period for a test to become positive

4.  You have not mentioned how long it has been since the exposure you mention since you have tested but if it has been more than the time periods I mentioned above, in my opinion and by recommendations from the WHO and CDC I see no need for further testing.  I see no reason related to the exposure that you describe to worry about having a baby with your wife.  EWH

Dear Dr. Hook, 

It has been 92 days after the exposure (my penis accidentally touched vagina for 5 seconds). I tested negative on 31, 55 and 92 days. All 4th gen test. Am I 100% sure that I don’t need more tests now? Can I have a baby with my wife now?

Thank you for the additional information.  Your tests at 55 and 92 days are absolutely conclusive.  You can be 100% confident that you do not have HIV from the encounter you described.  Certainly there is no reason to not have unprotected sex with your wife with the intent of having a baby.
