[Question #10541] Follow up - 10454

Hi Dr.

Thank you for all your support and having started this website.
Like many on this site I may be returning for closure and peace of mind from you guys due to my high anxiety and over worry.
Per my previous question I received unprotected oral from a male CSW. They said they are free of
All sti and on prep. And I followed up
Recently with them and see they are still operating their business as usual. (So I should have believed them but I needed reassurance).

I tested again at 31/32 days for hiv, syph, chlam, ghon and all came back negative. I did syphillis test at 6 week mark last week and negative. However I failed to inquire about HIV test at 6 weeks and now my anxiety is leading me to do HIV test for Closure at what would be 6.5 weeks now.

My questions (so I can finally calm my anxiety and worry)
-why do some sites state to test as far out as 12 weeks for HIV and syphillis ?
-what makes 6 weeks 100% conclusive ?
-how reliable would a HIV test at 4.5 weeks be?
-after  this I am not worry anymore? What is advice you offer patients to move on ?

Thank you

Welcome back, but I'm sorry you found it necessary. Directly to your new questions.

--why do some sites state to test as far out as 12 weeks for HIV and syphilis? Everything from overly conservative philosophy to not understanding the data. And the available data aren't all that precise for syphilis, although they are very conclusive for conclusive HIV results with the AgAb (4th generation) blood tests.

--what makes 6 weeks 100% conclusive? Assuming this is for HIV:  On this forum our advice is based on both the biological principles behind the test, plus published data https://pubmed.ncbi.nlm.nih.gov/29140890/ and CDC advice https://pubmed.ncbi.nlm.nih.gov/29140891/

--how reliable would a HIV test at 4.5 weeks be?  Around 98-99%.

--after  this I am not worry anymore? What is advice you offer patients to move on? Yes, no worries. Believe the data; and also do not rely only on test performance. In your other thread, we discussed several other reasons that you were at little or no risk. Your negative test results simply confirm the obvious. In my opinion, you didn't even need testing except for the reassurance it hopefully has provided you.

Finally, about that rabbit hole: If you find yourself compelled to keep searching online, at least limit your searches to professionally run or moderated sites, and avoid sites by and for people at risk (like Reddit, for example)--or at least understand the innumerable biases and BS they contain. Anybody can write anything on the web, and personal testimony about catching HIV is the worse possible evidence.

Best wishes--   HHH, MD

Thank you doctor for your promptness.

I’m sorry I had to come back as well. But being reassured and getting the expert opinion was needed.
I won’t go down any rabbit holes but just stick to this site.

I was asking about both syph and hiv (ag/ab) test being 100 % conclusive at 6 weeks. What makes them 100% conclusive at this time?

Thank you

Let me close out with another  question.
For some reason (could be my mind playing tricks)? I’ve felt a slight discomfort on my pubic hair area even after trimming  - couple isolated spots for a few weeks. No signs of lice, or pimples , etc. I’m assuming it is not std related?

The discomfort happens when sitting or sometimes walking. Feels like chafing in groin. Or poking hairs

Just want to rule out anything else for
Peace of mind

Neither HIV nor any other STD causes the sort of discomfort you describe. In addition, whenever someone suggests their own symptoms have a psychological origin ("could my mind be playing tricks?"), usually they are right!---
