[Question #10550] Forgot to Mention

Hello again Doctors,
I really appreciate your service and don’t want to abuse the service, but I have another question.

In my previous question about the oral encounter, I was ashamed to mention there was cocaine being used with a rolled up bill. I’m scared to have read about the transmission of Hep C or HIV from a contaminated bill. I saw no blood on it bill.

Also, this woman was in my bathroom and am scared they might have used my hair and face clippers or nail clippers. What are the odds there?

I forgot to mention a sore appeared in the centre of my tongue within 12 hours. It hurt a little. Then healed the next day and now almost a week later there is a little rough white line and a very faint circle around it. Is that too early as a symptom for anything?

Welcome back, but sorry you found it necessary. I reviewed your just-completed discussion with Dr. Hook and agree with all he said.

We are experts on STIs and sexual transmission, but not necessarily on nonsexual exposures. In this case, while there has been internet buzz about possible risks for transmission of blood borne viruses (HIV, HBV, HCV) from oral or nasal sharing of things like cigarets, cocaine snorting devices (including rolled currency), I am unaware that actual transmission of these viruses has ever been documented to occur in this situation. You can consider it zero risk and I advise against testing for these viruses on account of this event. And the situation is similar for shared razors, nail clippers, and the like -- here too, internet worries about it but no proved cases in which it ever was known to happen. Zero risk.

No infection of any kind can cause a sore or other symptoms under 24 hours after exposure, and no STI lesion can heal in under 24 hours. This must have been some sort of minor injury. Nothing is wrong.

I hope these comments are helpful. Let me know if anything isn't clear.

Thank you very much, I have friends who used to be non needle drug addicts and they had a bit of a laugh.

Do you agree too that I should get tested for the oral std issue? I think Dr.Hook says this for some people who’s anxiety can only be stopped with that.

I’ve read a lot on this forum and unless its a male to male encounter, testing is rarely recommended for oral with scientific logic.

What you have "read a lot" is exactly right:  from a medical or risk perspective, having no symptoms I would not advise testing on account of a single oral sex exposure. The only issue is whether you'll be more reassured by negative test results than by professional advice based on probabilities and statistics. If somehow I were in your situation, I wouldn't be tested. But you're free to do so.---

I’ll probably get tested. Last questions.

If you get 2 BJs from the same woman in the same night, brief, 30 seconds. Would that count as 2 exposures? Doubling the chance of exposure? Still low?

Does gonorrhoea live in the cervix? If you fingered a girl and she was infected, and you got close or touched the cervix. Then touched your mouth or even had your fingers in your mouth would that increase chances of transmission with oral?

Lastly. What are the symptoms to look out for in oral infection? I’ve had this sore/irritated throat, cough, have to clear it a lot. I also have a runny nose and smoke quite a bit.

Thanks again, service is incredible. Hope I don’t need it again.

No. of exposures:  There are no formal definitions. Who cares? Double an extremely low risk still means extremely low risk.

The cervix is exactly the site of genital infection in women. But fingering never transmits it. You'll never get oral gonorrhea except by performing oral sex.

Almost all oral gonorrhea causes no symptoms at all. In a few percent of cases, there is mild sore throat. It doesn't cause cough or runny nose. You have a cold and/or smoker's cough, or upper respiratory irritation from the cocaine.

That will be the last question about this exposure etc. Feel free to return if you test positive and we'll reimburse your posting fee.
