[Question #10622] Stage 3 fatty liver because of PEP?

Greetings doctor,
I am male 30 ethincity Indian. I am obese to begin with (175cms tall 103kg weight) . I first took pep of truvada and rilpivrine on Oct -2022 and another incident in may -2023 lead to another 30 day  of truvada and tivicay. On the first time I was very cautious of side effects and made sure to drink lot of water every day. But the second time I was bit careless and didn't drink water much. Both encounter were not that risky.. since it was my first time I got freaked out and opted for pep.. I tested negative for both encounter after 4 months after the incident. I got covid last month so wanted to access my lungs so my doctor ordered chest CT in that it was said "Grade 3 diffuse fatty infiltration of liver". My doctor is optimistic and told reducing weight can reverse this and prescribed udiliv 300 for 3 months.but I didn't tell him about pep though.. 

My question.
Is this because of pep??
If so is there any difference in treatment plan or should I continue with what my doctor prescribed and loose weight ( I didn't tell him about pep. He is a family friend and I don't want him to know)

Btw the current doctor is GP not liver specialist should I consult one??

I am obese for a very long time.. may be for more then 5 years.. after Covid lockdown my activities levels have shrinked so much. With work from home for the past 4 years I spent most of my time lying in bed.. there lot of day where I took literally 0 steps. And my average step count per day might be less the 500. Also before all this I was diagnosed with hypothyroidism and taking tyroxin 100mg daily..

One more thing when I was diagnosed with hypothyroidism I also had high blood cholesterol and was prescribed statins. This was before I knew there was a thing called pep.. so this is my full medical records related to this incident.

And I don't drink or smoke ever.. vegitarian from birth..but a huge consumer of McDonald's and Domino's

Welcome to our Forum. Thanks for your questions and for your confidence in our service.  I'll be glad to comment.  Your fatty liver is unlikely to be related to having taken PEP.  There are long list of possible side effects for all medications but this had not been a common problem for persons taking PEP. Further, PEP is episodic and if your fatty liver were due to medications, it should improve on stopping the medicine.  From what you say, your fatty liver is more likely to be related to your inactivity and obesity.  Fortunately, fatty liver is reversible with exercise and weight loss.  I would not worry that this is related to PEP

Thanks for the reply doctor.

Should I mention this pep episode to my doctor. If fatty liver is because of pep will the treatment plan differ??

As I said, it is quite unlikely that your fatty liver is due to PEP.

Your doctor should always know about any medications you take  EWH
