Dear doctor,
I would like to express my gratitude for the invaluable assistance you provided me in the past few months
I recommended your forum to a male friend, and now I find myself returning to seek guidance for him. Since he is not fluent in English, I will be acting as the translator to convey his concerns, if that's not an issue.

In brief, my friend suffers from OCD and is somewhat of a hypochondriac. He regularly tests for STDs, even when not sexually active. His last test in August for a full STD panel, including HIV and Gonorrhea, came back negative.
He is currently worried about a potential exposure to HIV during a recent encounter at a massage parlor. The sexual activity involved the CSW masturbating him and briefly performing unprotected oral sex (15-20 seconds) on his penis, followed by protected vaginal sex for 4-5 minutes. He inspected the condom afterward and found it to be intact.

Based on the advice I've received from this forum and from reading other posts, I tried to provide him with the best guidance. Here are the main points I shared with him (please correct me if I'm wrong):

1 - There was virtually no risk in this sexual activity, as he used a condom for vaginal sex, and insertive oral sex is considered zero risk for HIV even whitout a condom. He also doesn't know the CSW status, and most do not have HIV

2- The intact condom after sex suggests it did its job, as condom failures typically result in them breaking wide open

3 - Since he is on his 25th day post-exposure and has no symptoms, this further supports that there is likely no cause for concern.

4 - Given the minimal risk and absence of symptoms, testing is not necessary.

Since I'm no expert, he is seeking additional reassurance directly from you.
Could you please comment on this advice? Does he need testing at all, and what is the likelihood of him being infected from an exposure like this?

Welcome back to the forum -- on behalf of a friend. I have to note that the exposure you describe and your specific questions are suspiciously similar to your own in your two recent threads. In addition, we discourage asking questions on other users' behalf and my replies are very succinct and there will be no discussion. If your friend has additional questions, he will need to sign in with a new question and his own account (no problem with using your credit card, with your permission); and you certainly can be with him to translate as needed. If he does that, we will reimburse the charge for one of the two questions.

"Could you please comment on this advice? "Based on the nature of the exposure described, I agree entirely with the advice you gave your friend. That is, I agree exactly with all four of your statements about the risk of such an exposure and other details of your advice to him. 

"Does he need testing at all...?" No.

"What is the likelihood of him being infected from an exposure like this?" Zero for all practical purposes, as you have advised him.
