Dr, I have tested for all the popular STD’s (RapidSTDtesting.com 10 Test Panel) and come back clean but still suffer w swollen tender testicles, urethritis and burning after urination but no discharge other than when I am aroused and erect and some clear moisture comes out which I assume is normal. Tested negative for Trich and just did MyGen today. Is there anything else you recommend testing for that could be causing this? It is ruining my quality of life and highly uncomfortable. Am going w my wife tomorrow to have her test everything as well. This all began back in August (details in last question) Took Azithromycin, Cipro/Doxy and was much better before having sex w my wife when everything returned again. I’m 99% certain she has nothing. Need your guidance - pls help.

I'm responding this time, but I reviewed your recent discussion with Dr. Hook. I agree with all he said. With your recurrent symptoms after sex with your wife, you may indeed have an internal urogenital tract problem, likely prostatitis -- especially in few of past symptoms and MRI showing an enlarged prostate. (How old are you, by the way?) Further, your urologist has treated you in a manner suggesting s/he also thinks this is the likely problem. That your symptoms initially started 10 days after the semi-sexual event you described in your other thread supports the conclusion that no STD is responsible; that's too long for STD symptoms. And your negative STD tests are reliable; and as already discussed, the exposure described is not a possible source of an STD or any other infection. Finally, your symptoms don't fit with STDs: despite use of the term urethritis, you have no evidence your urethra is inflamed; the main symptom is discharge, and clear fluid is entirely normal in all males when sexually aroused. I also wonder if your testicles really are swollen and tender, or just uncomfortable -- which fits well with prostatitis.
Frankly, I see no need for your wife to be tested for STDs (assuming you are confident she has not had any other sex partners recently). There is no possibility you infected her, and no realistic possibility you acquired any infection from her. Almost certainly this is reactivation of an internal, preexisting problem -- like prostatitis.
As your urologist will confirm, it is not surprising your symptoms haven't improved with the antibiotics you have taken. The causes of prostatitis often are mysterious: usually no bacterial infection can be identified and antibiotics often are ineffective. They usually are tried in the hope they will help but often do not.
So my advice at this point is to return to your urologist; and that your wife doesn't need STD testing. And please be assured your massage experience a few weeks ago is not the source of any infection, even if that event had a possible minor role in triggering your prostate symptoms.

Probably it's clear in my comments above, but just to be certain you understand: It is plausible that sexual activity may trigger symptoms in men with underlying internal genital tract inflammation, like prostatitis. I'm confident your urologist will confirm this. But as also explained above, this doesn't mean the symptoms are caused by a newly acquired infection. And STDs don't cause prostatitis, or the sorts of symptoms you have described. I hope it's all clear. I would be interested to hear what further advice or treatment you receive from your urologist.

Thx for the detailed response Dr H. You really are very thoughtful and thorough and it’s much appreciated. My concern seems to be everything improves for the most part after taking things like Azith/Cipro/Doxy which I’m sure is likely due to their anti inflammatory effects but makes me wonder if some bacteria is causing it - thus my question on if I should test for things like MGen, ureaplasma etc.

It's true that doxy and azithro have anti-inflammatory properties. Bacterial prostatitis also is possible, but penetration of most antibiotics into prostate tissue is poor, which is why prostatitis often is treated for 6 weeks or more.---
