Since we do not provide direct medical care, we do not specifically advise for or against any sort of treatment, including PEP. However, PEP usually is not advised after an exposure like yours. I would not even recommend HIV testing. There has never been a proved case of HIV transmitted oral to penis. Based on surveys of people who thought they acquired HIV from oral sex (about which people often are wrong), CDC estimated the risk from receiving a BJ at one chance in 20,000, if the oral partner has HIV. That's equivalent to receiving BJs once daily by infected partners once daily for 55 years before infection might be likely. So you might have a conversation with your doctor before deciding whether to go ahead with PEP.

For much the same reason, testing for other STDs also is optional following fellatio as the only exposure. But a urine or urethral swab test for gonorrhea and chlamydia will be valid by Wednesday. Assuming no symptoms (discharge from the penis, painful urination), you can expect negative test results. Blood tests for syphilis and HIV will be valid in 6 weeks. However, these also are optional -- the risk of infection is near zero.

STDs do not travel through the body to cause symptoms at a site that wasn't exposed. Late syphilis or advanced HIV cause symptoms in many body sites, but no sooner than a few months after exposure. Your penile exposure by fellatio a few days ago cannot cause any symptoms of your mouth, throat, etc -- only of your penis or urethra.
