[Question #10768] Peculiar Case

Hi Doctors,
I recently received a a protected blowjob from a masseuse at an Asian massage spot.  She retrieved the condom from another room and asked if I preferred the normal looking condom versus what looked like a thin loose plastic covering. I chose the normal looking condom. She then put it on me and performed for about 2 minutes before removing and finishing with a handjob. My concerns are:

1. The condom was unwrapped - Could it have been old?
2. When she placed it on me she then wiped it down with a wet wipe then dried it while it was on me before performing. I assumed it was because she didn't want the oily substance in her mouth but what if she was cleaning it?
3. And whats making me the most anxious - if it was a previously used condom with previous ejaculate residue - is getting that on my penis a risk of HIV?
4.Should I be concerned about anything? 
5. I'm trying to also reconcile why she stopped a few times while performing to spit into a napkin.

Welcome to our Forum.  Thanks for your questions.  I'll be glad to comment.  Let me give you my "take" on the entire encounter- the fact that your partner used a condom had more to do with her efforts/desire to protect herself than to protect you.  To that end. she was more likely to use condoms which she had confidence in.  Let me also add that we have had many questions from clients over the years who worried that in interactions with causal partners, including commercial sex workers, that those persons were intentionally taking steps to infect others.  Despite the relative frequency of these concerns we have NEVER seen or heard of an instance where that is the case.  In my opinion, this makes perfect sense- a CSW who is intentionally infecting clients will not be in business long.  With these preliminary comments, let me address your specific questions:

1. The condom was unwrapped - Could it have been old?
Unlikely. I suspect by "old" you mean used.  If it were it would be misshapen and show signs of previous use.  Condoms are simply not expensive encoungh to re-use.

2. When she placed it on me she then wiped it down with a wet wipe then dried it while it was on me before performing. I assumed it was because she didn't want the oily substance in her mouth but what if she was cleaning it?
For all of the reasons alreadymentioned, I find this most unlikely.  I have never heard of someone re-using a condom, nor of cleaning used condoms.

3. And whats making me the most anxious - if it was a previously used condom with previous ejaculate residue - is getting that on my penis a risk of HIV?
I've already told you all the reasons I find this most unlikely.

4.Should I be concerned about anything? 
My sincere advice  would be not to worry about this encounter.  RAther I would consider it safe and no risk.  If I were in your situation, I would not be worried enough to even consider testing.

There may have been oral secretions or other things such as condom lubricant that she was spitting out

I urge you not to worry about this encounder.  I am confident was you describe was a safe, no risk sexual encounter.  EWH

Hi Dr. Hook,

Thank you very much for your prompt response. It definitely has helped alleviate my fears/anxiety. 

Just for clarification purposes and maybe to the benefit to other readers down the line - If for all intents and purposes a previously used condom is placed onto a penis, rubbing/applying bodily fluids onto your genitals does not constitute penetration- in this case genital secretion/sperm in the condom getting onto my erect penis would not be a risk for specifically HIV. Is that a correct assumption?

My apologies. My mind just goes to the extremes sometimes when anxious and I understand quite a few variables would have to line up i.e,...condom being old/re-used, it containing residue, the person before me being positive, the masseuse being negligent, the masseuse risking her own self. 

Outside of that, even if it had been unprotected and she were positive, this would have been a " near zero"  risk activity?

I think you are going down a rabllit hole.  The most important clarification I can provide to you is to say emphatically that I have never heard of someone using a condom twice - not in the sceintific literature, not in 40 years of sexual health focused practice and not from any colleagues.  This is, to my way of thingking paranoid thinking.

I will add the following FACTs, which is what we emphasize on the Forum:
1.  Exposure of genital secretions to the environment renders HIV non-infectious.
2. Indirect exposure to genital secretions such as when a person engaged in mutual masturbation gets genital secretions onto a partner, there is no risk of STI transmission, including HIV
3.   Transfer of genital secretions on a persons hands to another person is a no risk activity in terms of STI transmission.

I will not take part in these speculative "what if" questions- they serve no healthy purpose.

If your partner did have untreated HIV, the activities you describe would have still be virtually no risk events.
