[Question #10792] Urgent PEP Question! Oral?

Hello Doctor,
Heat of the moment I met up with a guy (I am also male) and gave him unprotected oral sex. He was uncut and produced a lot of pre-cum (pre seminal fluid). He deep throated me and I could feel his penis down my throat but also on my tongue and other areas of the mouth. He did not ejaculate in my mouth. I bit my cheek 2 days before the encounter pretty hard while eating. It was not actively bleeding during the encounter but there was a white sore in that area. I did not brush or floss for many hours before.
We deep kissed and he fingered me but nothing else.
I am worried about HIV. I asked him twice and he said he gets tested every 3 months (his profile said last test was October) and said “don’t worry”.
I am still worried what if he got infected since and thus has a high viral load. I am going to try to seek PEP. Is this reasonable? The deep throating particularly worries me because what if the tissue there is more susceptible and has less saliva.

Welcome to our Forum. Thanks for your questions and for your confidence in our service. I'll be glad to comment. The short answer to your question is that the exposures you describe were very, very low risk and I would not recommend PEP for you following the activities you described. In fact, if you asked me to prescribe it, I would decline to do so.
The chance that your partner had HIV is quite low. He not only told you that he gets tested regularly but told you when his last test was. The likelihood that he had acquired HIV since his last test is low- after all, he's avoided infection this far.
Receipt of oral sex, being fingered (masturbation) and kissing are no risk activities. There are no proven cases in which HIV has been acquired by any of these activities and that you had a recent place on your cheek that had been bitten does not change this in any way.
Finally, there have been cases of HIV acquired by performing oral sex on an untreated, infected partner but this is quite rare, estimated to occur about once in every 10,000 exposures to an untreated infected partner. In other words, if he had HIV, there is a 99.99% chance that you would not be infected by the exposure you describe.
Bottom, line, while PEP is always a personal decision, your risk is very, very low and I would not recommend PEP in this situation. In fat, the risk is so low that I probably would not even bother testing related to the activiites you describe.

Thanks Doctor. I’m glad to hear you don’t think this is a high risk and I don’t need PEP.
I actually just talked to a nurse and doctor at my local HIV testing clinic and they refused to give me PEP as well based on my encounter.
Just to confirm the deep throating and bite in cheek didn’t make a difference in risk for me giving this Man Unprotected oral sex?

Last question - if my results when I test next week for oral gonorrhoea and chlamydia come back negative, can I safely resume sexual encounters with a long term partner without fear of infecting them with STIS/HIV? They are aware of my encounter.

what is the accurate window for oral swab tests?

Yes, when your tests for oral gonorrhea and chlamydia are negative, as I anticipate they will be, it is safe to resume unprotected sexual encounters with your long term partner.
Had you been infected with gonorrhea or chlamydia, tests would be positive 3-5 days following your exposure.
