Hey doctor I have a couple more questions if you’d please help me out. 

Im currently now on day 14 of taking pep. 
(Truvada and isentress). Today on day 14  of taking Pep I forgot to take my pep on time by 30 Minutes late. The time I usually take it is 1:30pm and 1:30am daily. Today Instead of taking my  1:30pm pill on time I took it at 2:00pm making it 30 minutes late… 

My questions are as follow 

1. Does the 30 minute delayed dose of Truvada and isentress effect my treatment in preventing HIV at all? Whether it be minimal effect or substaintial or none at all?

2. Did this 30 min delay count as a missed dose? 

3. Is it  allowed to change the times I take pep if still taken in 24hr intervals? For example could I change the regime to 12:30pm and 12:30am instead of the original regime of 1:30pm and 1:30am without Effecting this effectiveness of the drugs treatment? 

No sugarcoating doc haha I want the answer straight up even if it’s bad.. I read a lot of stuff online that says missing a dose can greatly effect the chances of Pep working.

5. Would a couple hours of delayed late time affect pep effectiveness? Say i was suppose to take it at 1:30pm but instead took it at 5pm. Any effect whatsoever? Again minimal or substantial?

Patience, patience. We answer questions within 24 hours. This forum should never be a resource for medically urgent issues.

As it turns out, this isn't urgent at all. As we discussed last time, you didn't need PEP in the first place.

The exact timing of PEP doses doesn't matter very much. PEP is equally effective as long as the drugs are taken once a day, even if the times are 2-3 hours different. Having taken a dose at 1:30 pm, the next day's dose could be anywhere from 11 am to 4 pm. (Various resources advise sticking to an exact schedule not because it's highly imortant for PEP to be effective, but because a strict schedule often is the best way for people to remember to take the drug at all. Don't forget that PEP is most commonly used in people at true high risk, and many of them have memory difficulties, drug use, and so on.

I think those comments answer all four (not five) of your numbered questions. Let me know if anything isn't clear.

As to question 4 just to clarify it wouldn’t matter if I took the pill say at 5 pm rather than the original 1:30pm regime? Making it almost 3 and a half hours late? Just clarifying it since you said the next dose could be anywhere from 11am to 2pm in your last reply. 
Couple more questions
When I saw the escort I had sex with her with my underwear on (penis through the front pocket hole of the underwear) the escorts vaginal fluids would have got onto the underwear. So I decided I was going to throw it out when I got home. I put my Underwear and jacket in a bag to throw it out but I forgot to throw it out and it was sitting in my room. 
11 days after, which would be 11 days of my underwear and jacket sitting in a bag in my room my girlfriend picked up this bag and took out the underwear with her barehands! And washed them in the washing machine. 
My question is 
1. Is it possible my girlfriend could contract hiv from touching the underwear? Since it had the escorts vaginal fluids on it and she touched it with her bare hands.

2. Should I throw out my jacket and underwear or can I keep it since I washed it or is it mandatory I throw it out since it could maybe have stis on it? 
3. Should she seek testing?

You can find official advice about PEP on various websites, including CDC (www.cdc.gov). They all advise that if you miss a dose at the scheduled time, take it whenever you remember it, as long as it's the same day. Being 3-4 hr late (or even missing a dose entirely every few days) makes no known difference in protection.

HIV is not transmitted by the things you mention -- never. 1) No, your gf could not contract HIV in this manner. 2) There is no need to discard your jacket or underwear. Washing it is sufficient -- and even that is a good idea just for cleanliness, not making any difference in infection risk. 3) Your gf should not seek HIV testing.

Your questions indicate you have some peculiar ideas about HIV transmission risk. The virus is never transmitted sexually except by intercourse; or by infected blood introduced inside the body. It is never transmitted through contaminated clothing or objects.

Before you ask anything else, please go back and re-read every word of our discussion in your previous thread two weeks ago. Thank you.

Ok thank you for your advice doctor. I want to ask some closing questions and hopefully i will put this all aside and just wait for testing. 
My girlfriend also smoked my vape right after i hit it. Like 10 seconds after it was in my mouth.  Before I gave her the vape I wiped the Mouth piece of the vape throughly with my tee shirt. 
Question is 
1. Can she get hiv from smoking the vape right after it was in my mouth? 
2.did me wiping the mouth piece of the vape prevent any transmission if any?
3. Should she get tested in 3 months time just in case 
4.is it even possible to get hiv this way?
5.since I am on PEP does that make me unable to transmit to virus if I do have it? Since PeP suppresses the virus?

I forgot to add that my lips are very chapped and cracked from cold weather and I did bite my lips earlier yesterday but no blood whatsoever.

1,2,4. Answered above. Was there something you didn't understand about "The virus is never transmitted sexually except by intercourse; or by infected blood introduced inside the body. It is never transmitted through contaminated clothing or objects." And not by shared vaping equipment, glasses, cups, etc.
3. She should not be tested.
5. Almost certainly people on PEP cannot transmit the virus. And as discussed repeatedly, there is no chance you were infected anyway. Why is that so hard to understand and believe?
