[Question #10887] Exposure and test for HIV and Syphilis

Dear Sir, 
I am a Indian residing in the Middle East. During the recent past 7 months, I have had multiple sexual intercourses (protected vaginally intercourse) and six times unprotected blowjobs. 
During my last intercourse, my condom broke unfortunately (this happened on the 5th December 2023). After that encounter, I have never had any sexual affairs at all. Now, I am afraid I may have been exposed to HIV or Syphilis. 
I have taken two HIV tests (one early detection test(RNA PCR for HIV1) on the 24th December 2023 and another Dual Combo Test(P24 Antigen, HIV1/2 Antibody detection) on the 16th January 2024, and both came negative. Also I did a RPR test for Syphilis, which came negative. Now can I rest assured that I am in the clear from HIV and Syphilis? I have a one year old kid and I feel very ashamed and scared to even go near him thinking I might pass on any infection due to my negligence. Kindly help.

Welcome to our Forum. Thanks for your questions and your implied confidence in our service.  I'll be glad to comment.

I'm sorry to hear of your condom break- this happens about 1% of the time condoms are used.  Please contiunue to use condoms going forward.

Your test results PROVE that you did not acquire HIV.  Most experts accept HIV PCR test results any time more than 11-12 days following an encounter as conclusive although many also suggest confirming that negative result 4 or more weeks after exposure for reassurance.  Your PCR test result, combined with the results of your 4th generation test more than 4 weeks after the encounter prove that you were not infected.  

Your risk of syphilis is very low- few commercial sex workers have syphilis (or HIV) and an RPR at 4 weeks is very strong evidence that yu were not infected although many experts would suggest testing at 6 weeks to be entirely sure you were not infected.

The chance that you acquired HIV or syphilis is virtually zero.  Even if you did, there is NO RISK of infecting your child through your activities of normal living, including hugging and kissing.  I would not worry at all about interacting with him.  

Please don't worry and please don't deprive your son of your affection.  EWH

Thank you for your quick response. So I can consider myself 100% free of HIV and Syphilis, and do not need to take any more tests, right? Your confirmation to this will mean a lot to me.

Dear Sir, 

I noticed a small SINGLE pimple like formation (1.5mm) on the foreskin of my penis. It has been there for more than 2 weeks now. Twice as of date, there was a VERY VERY mild itch on it. No watery bubble or DISCHARGE. Now I am afraid this may be Genital Herpes. Can you kindly clarify 
A. How long will a genital Herpes blister last? Can it last 2 weeks or more without change in the appearance? 
B. If I do have it, will I pass it on to my kid when he sits on my lap or sleeps on my lap? 
C. Will it spread when I wash my clothes together with the rest of the family or use common kitchen utensils? 
Your response will help me a great lot sir, I lost my peace of mind and am worried about my kid and family.

A herpes lesion would not remain resent for two week and would not appear as a "pimple"

A. How long will a genital Herpes blister last? Can it last 2 weeks or more without change in the appearance? 
The blisters of herpes break open to form shallow ulcers within a day or two of appearing

B. If I do have it, will I pass it on to my kid when he sits on my lap or sleeps on my lap? 
Herpes cannot be passed on in the way you describe

C. Will it spread when I wash my clothes together with the rest of the family or use common kitchen utensils?
No

This in no way suggests herpes.  There are many common, none-STI dermatologic problems that can appear on the penis.  I suspect that is what you are seeing.  If you are concerned, see a dermatologist
