[Question #10900] Finger Frolicking and Oral

Hello.  This past Sunday Jan. 21 I met up with a girl.  Filipino age 40.  She has been in US for 10 years.  I kissed her, played with her breasts and fingered her.  She performed oral on me.  I am concerned because my nail bed had a minor nick / cut / irritation.  At the time of inserting it could barely be seen.  It hurt a bit if you really squeezed it.

I since have made it worse by picking at it.  I actually made it bleed.

Symptoms-
Yesterday (very late in the day) I appeared to get minor angular cheilitis. I am pretty sure.  I have had it before.  This scared me.

Questions

1. Isn’t it too soon to be Ars at 4 days post risk? (Almost 5 days)

2. Do I need testing from what I described regarding the fingering with a nick?

3.  Is unprotected receipt of a bj a risk for hiv?

4.  Can I go back to unprotected sex with my normal partner?

5.  Do I need Hcv testing?

6.  Am I doomed?

..lastly I had one or two warts removed from my genitalia over the years if that matters.

This event was a huge mistake.

Thanks

Welcome to our Forum. Thank you for your questions.  I'll be glad to comment.  The encounter you describe was no risk for HIV and close to no risk for other STIs. Your partner probably did not have HIV and even if she did, HIV is not acquired as a result of masturbating a partner, receiving oral sex, or playing with a partner's breasts.  The nick on your nail bed makes no difference, even if it was recent or bleeding.  To answer your specific questions

1. Isn’t it too soon to be Ars at 4 days post risk? (Almost 5 days)
Yes, it is far too early for this to be the ARS. If the lesions is angular chelitis, that would not necessarily be a sign of ARS- most persons with AC do not have HIV.  It can however be a sign of anxiety, reflecting repetitively licking the corner of one's mouth

2. Do I need testing from what I described regarding the fingering with a nick?
I see no medical or scientific reason for testing

3.  Is unprotected receipt of a bj a risk for hiv?
No, no one has ever gotten HIV from receipt of a BJ

4.  Can I go back to unprotected sex with my normal partner?
Certainly

5.  Do I need Hcv testing?
Absolutely not.  FYI hepatitis C is almost never an STI and when it is, it is related to receipt of receptive rectal intercourse, something that you did not do

6.  Am I doomed?
Certainly not from the encounter you describe

Please don't worry.  EWH

Dr,
Additional info.

I was recently hiv tested  at almost 9 weeks post risk with a 4th gen test after kissing a gal for 20 minutes.  The results were negative.

I have struggled with mouth, coatings and even had thrush before at least twice. Doctors have struggled to figure out the problem.

My only concern was your response to question one. You said:

“ yes, it is far too early for this to be the ARS.  If The lesion is angular cheilitis that would not necessarily be a sign of ARS. Most persons with angular cheilitis do not have HIV. It can, however, be a sign of anxiety, reflecting, repetitively, licking the corner of one’s mouth. .”

1.  Does that mean four or five days post  risk is too early for the angular cheilitis to be ARS. (If that’s what I have ?

2. What do you mean if you have AC it’s “not necessarily”. ARS?

3. What is the ARS timing?

4. Can I sleep unprotected with my partner ?  We have a date tonight.

Thank you for your time

You seem to misunderstand how HIV is transmitted.  HIV is not transmitted by kissing of any sort.  Mouth sores, gum disease, etc do not change this fact.

1.  Too soon means that the ARS does not occur at five days Rolland exposure.  Further, as already explained, angular chelitis is not a sign of HIV, including ARS.
2.  See above
3.  The ARS occurs 10-21 days following an exposure.
4.  Absolutely

Dr.

Reminder- my risk was fingering with a minor cuticle cut / nick.  I also received an unprotected blowjob.

Because day 5 I had angular cheliitis I went and had a rapid 4th gen test on day 10.  It was negative.

Today I defecated when I woke up.  My stool was normal. About 30 minutes latter I began to have anxiety.  I returned to the bathroom and had an episode of diarrhea.  Now I feel ok.

1.  Did my negative test rule out the angular cheilitis was Ars?

2.  Does todays diarrhea (12 days post risk) sound like Ars?

3. Does diarrhea happen in isolation or with other symptoms?

4. Can I continue unprotected sex with my normal partner?

5. Should I be worried that 5 days after the angular cheilitis I had diarrhea?  Does this sound like ARS.

6.  Do I need further testing?

7.  Am I worrying about nothing?

Thanks.

I forgot to mention I went to the doctor because my lower back hurt and my testicle on the same side as the back pain felt odd.
