I am a 27-year-old male. I made a huge mistake yesterday and am now very anxious. I really need your help.
Yesterday, I made an appointment with an escort I found online. She is a young woman who appears to be 25 or younger. We showered together, during which she used her hands to clean my body, including my genital area and anus.
After that, we kissed each other deeply for about 10 minutes. She began to kiss, lick, and suck my body, covering all over, including sucking my scrotum. However, her mouth did not touch my penis.
Then, I started to kiss, lick, and suck her body. I performed oral sex on her for about 10 minutes and also sucked and nibbled on her breasts and nipples for 5-10 minutes.
We did not have vaginal sex.
Here is the worrying part: I currently have three canker sores in my mouth, which are painful. This has made me very worried. I have the following questions:
1. If the escort had sores or open wounds in her mouth, am I at risk of HIV from kissing her?
2. If there were small cuts near her genital area, am I at risk of HIV from performing oral sex on her?
3. If there were small cuts on her breasts or around her nipples, am I at risk of HIV from licking, sucking, and nibbling her breasts and nipples?
4. Do any other activities I mentioned pose a risk for HIV?
I really want to move on from this event. I am filled with regret and need your help. If anything I've mentioned is unclear, please let me know. Thank you for your help.

Welcome to our Forum. Thanks for your questions, as well as the detailed description of your encounter. I'll be glad to comment.
This was a very low, close to no risk encounter. I'll provide answers to your specific questions below but let me start by pointing out that very few commercial sex workers have HIV and even when they do, most encounters with infected untreated persons with HIV do not lead to infection. Some encounters are lower risk than others- the encounters you describe were amongst the lowest risk. HIV is transmitted primarily by genital-genital or ano-genital contacts. Oral sex is very low risk and the highest estimates of risk from oral sex are that less than 1 in 10,000 exposures will lead to infection (in other words, infections are not transmitted 99.99% of the time that oral sex is the only encounter. In answer to your specific questions:
1. If the escort had sores or open wounds in her mouth, am I at risk of HIV from kissing her?
No! HIV is not transmitted by kissing, even deep kissing and even kissing when one or both persons has mouth sores, dental problems or gum disease.
2. If there were small cuts near her genital area, am I at risk of HIV from performing oral sex on her?
No, this would not change the close to no risk nature of the estimate you describe
3. If there were small cuts on her breasts or around her nipples, am I at risk of HIV from licking, sucking, and nibbling her breasts and nipples?
No! This too was a no risk activity
4. Do any other activities I mentioned pose a risk for HIV?
No
If I were you, I would not bother with testing related to the activities you describe. If you choose to test to relieve your anxieties, at 4 weeks results of a 4th generation test will be more than 99% conclusive and at 6 weeks results will be entirely conclusive. Should you choose to test, I am confident that your tests will be negative. EWH

Forgive me if I am repeating myself. I just want to double-check whether you noticed that I had three painful canker sores during our encounter (and I still have them now). I am not sure if the canker sores bled or not. Do my canker sores (assuming they bled during the encounter) change your assessment? Can I still be confident that my risk is extremely low?

I didn’t notice and understood that you had three canker sores at the time of your encounter. This is no way changes my ass assessment. This was still a virtually no risk encounter. Please don’t worry. EWH.---
