Hi
I had vaginal sex with Escort in a stripper club in a VIP room, while having it my condoms slipped off which I didn't noticed. This is my first time sex in my life. After 28th day i took a test of 4th generation (Ab/Ag) and then at 55th day too the same test and both are negative with quest diagnostic lab. I too confirmed with her she told that there she is negative but doesn't want to give to show her report. Then i bit relaxed after 55th day
On the 85th day I got fever, mild sore throat, dry lips, running nose, increased heart beat, blurred vision, difficult in swallowing and head agitation.
My queries:
1. I have a weakened immune and under immunosuppression therapy. I agree if weak immune system antigen will spread quickly and antibodies will delayed and 4th generation will detect antigens and antibodies. Do you see any cases where it happened after 55th day? I know CDC recommendation itself is 45 days conclusive. Just want to confirm these with experts.
2. Is there any chance that I will be getting the ARS after this long time? Do I need to a re-test now to confirm that I don't have HIV or not required? I read somewhere that ARS occurs within a month itself.
I hope retest is not required and don't have any infection.

You are in the clear. Despite being on immunosuppressive therapy you have proven that you were not infected with your testing. If your antibody production had been delayed or reduced, the p24 antigen levels in your body would be very high and certainly detected by your tests. Thus, In reply to your specific questions:

See above. Even among immunosuppressed persons I have never seen nor heard of anyone with a 4th generation test which was positive by day 45- in fact over 99% are positive by day 28. Believe your results

No, you do not have HIV. Not everyone who acquires HIV experiences the ARS. The ARS is due to the interaction of antibodies and the virus itself. Only about 30-40% of persons who acquire HIV develop the ARS. That said, you can be confident that you do not have HIV. No one develops the ARS more than 30 days after an exposure.
Please don't worry. There is no reason for additional testing. You did not acquire HIV from the exposure you described. EWH

Few question our of curiosity
1. I know 4th generation is conclusive. But on 3rd generation (antibody) window period is 90 days, but will this applicable to both immune suppression patient as well as normal patient. I know it depends on the antibody produced based on immune suppression. But by 90 days will all develop antibodies expect chemo patient. I am at moderate immunesuppression drugs.
2. I agree ARS is due to the interaction of antibodies and the virus itself. but incase of immune suppression patient it will take more time to develop, so ARS also will be delayed right?
3. I am having pain during ejaculation, this is my first time sex( was virigin before this one). I tested chlamydia gonorrhea syphilis trichomoniasis, HSV I and II all are negative. Took doxycycline for 7 days still some pain is there during ejaculation. Is it something to worry about. ?
This is only worrying factor as of now as pain during ejaculation

Thanks for your follow up questions. Before I answer your specific question, let me point out less than half of person who acquire HIV experience the ARS. Thus, the absence of the ARS is a little relevance while if symptoms potentially due to the ARS are present, testing will help to determine whether the symptoms are related to HIV or not. Onto your follow-up questions:
1. This is a “what if” question, which serves little purpose and is difficult to provide a specific answer for because there are so many different levels of immune suppression. By and large however, if you are on immune suppressive therapy and not suffering from frequent infections, your third generation HIV antibody test results would certainly be conclusive at any time more than eight weeks an exposure.
2. Correct date suggested if the ARS is going to occur it will occur within 30 days of an exposure.

I trust you doctor and I am confident that I don't have HIV, since i took 4th gen test on 28th and 55th day and no need for further testing
I am getting frequent UTI infections, i hope this is not due to the that HIV and it is due to some other reasons.
Also can i resume my sexual life as i am not planning to take another test anymore.
These all symptoms occurs after 85th day so i hope it is related to some other infection.

Also I am having symptoms like blurred vision, increased rate and diarrhea . Is this something to worrry about or it is some other infection

I am pleased to hear that you now accept the fact that you do not have HIV. It is time to move on. There is no reason to hesitate about having unprotected sex with your regular partner related to the encounter you described.
Blurred vision and increased frequency of diarrhea are not related to the encounter you described and may be related to a non-STI infection. I suggest you work with your regular doctor to address their cause so that they do not inconvenience you.
