[Question #10944] Std and epididymitis

Hi
Well I didn't think I would ever be back on here but I am
I had 2 brief oral exposures msm many months ago
On your advise -mainly my paranoia- I got tested for clam and gon in the first week in Jan approx 4 weeks ago and the brief oral was 7 to 10 weeks ago
Never had any symptoms of stds
No discharge, no pain urinating etc
To this day I have no symptoms of anything down there
My job is construction and very hard and I'm getting older 60
Last week I ached all over after a hard week, particularly in my groin area
I went back to work Monday this week and by this past Wednesday I felt a bad pain in my right testicle
I felt it and it ha become enlarged
It was painful and swollen and was very hard on the bottom
I first say a Dr and he examined me
And said it could be a cyst
He sent me to urinology department and they told me that it could be epididymitis and asked me if I were sexually active I said yes but only with my wife
They gave me ciprofloxasin and took blood tests
I googled epididymitis- which I have never heard of before - and it said its caused by a std of which me and my partner have zero symptoms
They just called me for a scan and it sounds urgent
Please help

Welcome back, but I'm sorry you found it necessary. I reviewed your recent discussion with Dr. Hook and agree with all he said. There was little chance of any STD from the non-marital sexual exposure you described.
Your symptoms indeed are typical for acute epididymitis, i.e. testicular infection. In men over age 35, most cases are due to common UTI type bacteria like E. coli and similar organisms -- and the older one is, the more likely such bacteria are the cause and STDs unlikely. At age 60, the possibility of an STD as the cause is very low. It seems you've been misled by your online searching. STDs definitely are rare as the cause at your age. And as discussed with Dr. Hook, absence of urethral discharge and painful urination are solid evidence you didn't have gonorrhea; and chlamydia is almost never acquired by oral sex. So although I understand your concern given the timing not long after your oral sex event, that's all it is -- a coincidence.
You don't mention diagnostic tests for infection. Did your urologist not order at least a urine test for UTI bacteria? For reassurance, you also could have had a urine test for gonorrhea and chlamydia: as already noted, there is no realistic chance of an STD as the cause of your epididymitis, but I think you will agree it would be reassuring for you to know you tested negative. If you had been honest about your sexual exposure, probably the initial doctor and/or the urologist would have tested for them. You still can do that, because although ciprofloxacin is good treatment for most UTI bacteria, it doesn't work at all against chlamydia and is unreliable against gonorrhea. So a test for them still would be valid. If done, you can expect a negative result.
We are not experts in the urologic aspects of epididymitis. Most likely the reason for the "scan" (CT scan? ultrasound?) is to be sure the epididymitis isn't developing into an abscess. This is something to discuss with the urologist. In the meantime, despite the coincidence in timing, I am very confident this problem has absolutely nothing to do with the oral sex events several weeks ago.

I missed your follow-up comment about negative gonorrhea/chlamydia testing. I'm glad to hear it -- that result reinforces both Dr. Hook's advice and my own. You can be 100% certain your epididymitis has nothing to do with the sexual exposures several weeks ago. And no need tell your urologist or to test again for gonorrhea/chlamydia.---

As I had never heard of epididymitis, I was taken aback by the fact it could be a std
But the thing that intrigues me is that although I had a swollen and painful testicle and I surely did have epididymitis, how come if I did have a uti I didn't have any pain or discharge from penis
Makes no sense

This isn't unusual at all -- and in fact it makes perfect sense. Many UTI's cause no symptoms at all, and many if not most non-STD epididymitis occurs without typical UTI symptoms. Men with UTI symptoms typically seek care and are cured before epiddiymits can develop. It's also a good possibility you have a focus of infection in your prostate, which usually causes no symptoms. Discuss all this with your urologist. I expect s/he will confirm there's nothing unusual about this situation.---

They took a blood test about 3 days ago
Would that show up any infections

There is no blood test likely to help diagnose the cause of epididymitis or to diagnose other possibilities for the problem. My best guess is that the test(s) were those commonly done for people with any infection -- a CBC (complete blood count) can help judge the severity of an infection, and perhaps routine blood chemistries to screen for various health issues that become common with mature age. Here too, ask your doctor(s).
