[Question #10949] HIV risk

Hi Doctor Hunter
 I am a straight male from Turkey,38 years old,circumsized. After years of OCD and HIV anxiety and tons of search, readings,having tests, etc. I am still having issues about having sexual realtionships with women. Because of the antidepressant i use ,sometimes i lose erection while having sex with condom. In October 2023 while i was having a vaginal intercouse with a lady with condom, i lost my erection and continued to sex without condom for about 10 seconds,then i ejaculated(that was a one time brief sex only) Because of my knowledge about HIV, which i totally learned them from your answers in forums, througout the years, i didn't make so much panic and started to wait to have a regular test that i have routinly once a year.
One month after i had an unprotected oral sex from another woman. That was all.  We didnt have sex because she was on her period.
Than one week ago i had  sex with another woman.(vaginal intercourse) It was protected at first but i lost my erection again and continued without condom, it lasted ,again, only 5-10 second, then i ejaculated. Those three ladies are straight white females, (not CSWs) that i met in social daily life.They are in their 30s. Also those two intercouse included brief unprotected oral sex  but i am not worrying about it too much.
4 days after last intercourse i had a runny nose but i didn't last long, only 2 days. I didnt have any other symptoms yet.I tried to protect myself, i tried to use condom but at some point i couldnt achieve.
Now I started to google everything about the risks of acquiring HIV through vaginal sex from female to male. But i cannot convince myself about the risks.Can you please evaluate my risks. I need to hear something from you ,to ease my mind until having the test at 6 weeks.

Welcome to the Forum. Thanks your your questions, as well as for reviewing interactions with others we have had in the past.  We leave them available so persons can do just as you have and review the information we provide.  

I agree with you that the encounters you describe were low risk for HIV- HIV is relatively rare in women, your unprotected exposures were very brief, and even if one of them was infected, which is unlikely, your risk for infection is quite low.  The symptoms you describe in no way suggest HIV.  I would not be worried and agree with your plan to wait to test.  FYI and acknowledging your OCD, if you wish, testing 4 weeks following the last exposure will provide highly reliable although not perfect results.  At 4 weeks following acquistion of HIV more than 99% of infections will be detected using 4th generation, combination HIV antigen/antibody tests,  Neither of us on the Forum has ever seen anyone with a negative test at 4 weeks go on to become positive.

Thank you for your response sir, last thing i want to ask, should i wait a %99 negative result  after these incidents at 6 weeks hiv test? and soemhow if i was in your clinic after this story would you prescribe pep to me after a brief unprotected vaginal sex?

I really appriciate the work you make on the internet for several years. Your responses makes much more easier to understand the things we are facing. Thank you.

Remember, you are inquiring about not one but 3 encounters occurring over a period of several months.  Thus, I would not recommend PEP for you but that is always a personal choice.  PEP must be started within 72 hours of exposure however so it would only be relevant for your last exposure, if at all.  

I would believe results of testing performed 4 weeks after the last exposure.  EWH.

Hi again Doc
I am on my 22 day of my last brief unprotected vaginal exposure and waiting to test at 6 week for hiv. But today i noticed a brown bump on my penis shaft. I think it is a genital wart. I know it because 10 years ago i had 2 warts on the same area and the dermatologist had removed them( told me those are genital warts)I didnt ever have any wart since then. I know having a wart is not a big deal but it made me upset.My anxiety is increasing and i dont know if this single wart is because of stress or sth? My plan is to see the dermatologist next week and take the tests at 4 weeks. Do you think this situation increase my risk of hiv or other stds? Thank you for the last reply. I appriciate

As you know, this will be my final response.  Several comments.

Your exposure was low risk.  Testing at 4 weeks rather than 6 weeks will provide an answer that is more than 99% conclusive.  Alternatively your could test with an HIV RNA PCR test now and get an answer that is conclusive (these tests are considered conclusive any time more than 11 days after an exposure).

I cannot tell you if the bump on your penis is a wart or something else.  Seeing a dermatologist is a good idea.  FYI, it typically takes more than 2-3 months following an exposure for warts to appear.

I agree with your plans to test at 4 weeks.  If the lesion is a wart, it does not change your very low risk for HIV or other STIs.
