This is a follow up from my posted question # 10994. My doctor declined to do a oral swab, because he feels it is anxiety and esophagitis, which I was prescribe omeprazole and sucralfate.  The medicine has helped and my "Globus sensation" has gone down.  I have been looking at the inside of my mouth daily since "exposure"  almost 3 months ago and today I was able to find some white pus like spots on my tonsils, which I took a Q-tip and was able to wipe away.  What is bothering me, is I haven't had sex with my wife, but once since then, in fear of affecting her.  Her period was a week late afterwards and her period just came on time this month, but her cramping is hurting more than normal.  I have done std panel at around 38 days, including Trich and 3 more Oraquick tests at 8, 9 and 11 weeks.  All was negative, only thing I have not tested was Oral Swab for GC. Current Symptoms: Dry mouth/throat in the mornings and a couple tiny white pus spots in my tonsils (They don't resemble the streak spots in google pictures)

1.  Do white pus spots develop this late in Oral G & C?  I didn't notice any spots until 11 weeks after. 

2. My wife hasn't expressed any issues, besides the late period and more painful cramping this time around, does PID occur this fast? 

3.  If oral G&C was causing my esophagitis and globus effect, I am assuming that the medication of omeprazole and sucraldate would not have worked?

4. Is tonsilitis a symptom of Oral G&C?

Welcome back, but I'm sorry you found the need. It's so unnecessary. I read your recent discussion with Dr. Hook and also scanned your previous threads as well. It is clear you are psychologically conflicted about your sexual choices and have tendency to wildly overestimate your risks for STD.

It seems there is no chance at all you had a sexual event you don't even remember; and if you did perform oral sex on someone, your STD risk was minimal, nearly zero. I agree with your doctor's advice and decision not have done a throat test for gonorrhea and chlamydia. And even if you had either of these STDs, it is now gone. Oral gonorrhea (and the very rare case of oral chlamydia) are cleared by the immune system within 6 weeks.  To your specific questions:

1. No, neither of these causes white spots in the mouth.
2. No STD is a likely cause of late periods, which occur all the time in entirely healthy women. PID doesn't cause this.
3. Neither gonorrhea nor chlamydia has been known to cause esophagitis.
4. Gonorrhea can cause tonsillitis, but rarely does; over 90% of oral gonorrhea causes no symptoms. Almost all tonsillitis is caused by garden variety cold viruses.

Do your best to move on without worry.

Hi Dr. Handsfield,

You’re right, I do grossly overestimate. Your service does help me though, especially mentally. I appreciate that.

1. I understand you have extensive knowledge on DGI, I assume since I haven’t had any symptoms besides my throat issue, I have nothing to worry about? It doesn’t come later? I am 86 days post exposure.

2. I assume Cunnilingus as the giver and receiver are both lowest tier for giving GC?

3. 45 days post exposure, I had a 1 pimple on my upper stomach. Now I have a few on my chest and neck, about 10 spread apart. I am worried about molluscum, everything. My pimples are super tiny, we are talking 0.2mm of size and hard with no pus and no itch. I can’t find on google a timeline from start to finish, can you elaborate a little on MC and the timeline?

4.  I understand Oraquick is your least fav test, but I have taken 4 (day 56, 63, 80 & 86) I assume this is conclusive?

1. DGI doesn't behave at all like you have described. It's not a possible explanation of any of your symptoms.
2. Yes, both.
3. So you have a few pimples. Why does it even enter your mind that they could have anything to do with the sexual events you are worried about? And MC only occurs at sites where it is massaged into the skin, which undoubtedly did not include the locations you describe. You can google MC to see plenty of images to compare with what you have noticed.
4. There is no chance you have HIV so no testing at all was needed, and any and all negative results confirm you don't have it. However, the oral fluids test never becomes positive in up to 5% of people with HIV. If you really feel you need a test that by itself is 100% conclusive, you'll need a lab based test, such as an AgAb (4th generation) blood test.

I ended up seeing the Derm and was told it was mild folliculitis. I also took the 4th gen today, just to put my mind at ease, since those results will be conclusive. I see my PCP tomorrow and will basically push for a throat swab so I can get my mind away from it.
Current symptom: Sore throat, mild tonsil swelling

1. I did not start having any symptoms besides this throat issue 6 weeks after my blackout event. I was perfectly fine for the first 6 weeks. I read several times on this forum that if Oral G,C symptoms occur it would be in the first 2 weeks of transmission? I just wanted to confirm if this is still your stance. 

2. My PCP said it was esophagitis from my acid reflux. The medicine prescribe has help the “globus effect” go away, however now my throat has been kind of sore. If it was Oral GC, does the symptoms linger this way? I have had this throat issue for the last 6 weeks.

3. Since my blackout, I have only performed oral sex on my wife one time, due to fear, this was 5 weeks post blackout and I didn’t have a throat issue at that time. I understand cunnilingus is low transmission and it is repeated in the forum, but can you elaborate a little? 

I appreciate you, Dr. Hook and Dr. Warren. Reading the 100s of posts on your forum, I realized 50%+ of the posts are people in my position of guilt, anxiety and reddit rabbit hole reading. Thank you.

1. Oral gonorrhea rarely causes symptoms and chlamydia never does. When gonorrhea does cause symptoms, it usually happens within a couple of days.
2. You have your doctor's diagnosis. Gonorrhea and chlamydia are not possibilities. Let it go!
3. What's to elaborate about "extremely rare if ever"? There are no precise data.

Please note the forum policy against repeated questions, especially when anxiety driven. This being your second, it must be your last about your recent non-exposure event and your STD fears as a result. In addition, let's make it your last question about STD risks from cunnilingus, which has been addressed in all five of your threads. In the event of future concerns along these lines, just re-read the replies in those threads; they contain the answers to any other questions you are likely to have in the future. The policy against repeat questions is based on compassion, not criticism; experience shows that repeated answers tend to prolong rather than relieving anxiety. Simply hearing the facts and probabilities another time rarely helps. In addition, ASHA is not keen on getting paid for questions with obvious answers. Finally, such questions have little educational value for other users, one of the forum's main goals. Thank you for your understanding.
