[Question #11083] Brief Unprotected sex with escort
17 months ago
|
Hello. I just left an appointment with a Korean escort that offers unprotected vaginal sex. I planned on using a condom, but in the moment she took my penis and inserted it in her vagina. There was a lot of lubricant. I didn’t notice any sores. The intercourse lasted for maybe two minutes. I freaked out and asked her if she was disease-free and she said that she is. She also said that she gets tested “regularly”. Still, the fact that she does this with her clients is making me extremely nervous.
What should I do?

Welcome to our Forum. Thanks for your questions. I'll be glad to comment. No single sexual encounter with a new partner is very high risk for acquistion of any STI, including HIV. That's because most people, even most CSWs do not have STIs and, even when they do, most single exposures to infected partners do not result in transmission of infection. Transmission rates following a single exposure to an infected partner on average range from about 20% following exposure to a partner with gonorrhea or chlamydia to less than 1 in 2000 for exposure to a partner with untreated HIV.
In general, most CSWs, particularly those "higher end" persons and persons who work by appointment or in brothels tend to check regularly and to not be infected (infecting partners is bad for business).
---Thus, the odds are in your favor. Testing for the most common STIs (gonorrhea and chlamydia) can provide additional assurance that you were no t infected. You could have a urine test for gonorrhea and chlamydia at 3-4 days after your exposure and get reliable results. OTOH, for less common diseases such as syphilis or HIV, it takes longer for reliable results, typically as long as 6 weeks for the blood tests. Testing is always a personal choice and depends on your level of concern.
I hope that this information is helpful. EWH

Thank you Doctor.
Last questions for you:
- would you recommend PEP ?
- given your extensive experience and the fact that this was a fairly brief encounter (2 mins max) do you think I should expect a negative result for HIV?

While once again this is a personal decision, I see no reason for you to pursue HIV PEP. Statistically your risk for infected is less and probably far less than 1 in 20,000 (i.e. chances are more than 99.99% that you are not going to get HIV from this encounter). Further if you were to take PEP, then you would have to wait an additional month until you could reliably test for HIV. If you approached me in my clinic requesting HIV PEP I would not be willing to prescribe it. EWH---

Thank you. Given the scenario I described, would you expect my HIV status to remain negative ? (Ironically before this encounter I tested for all stds and was happy that they were all negative)

We provide up to 3 responses to each client's questions. This will be my 3rd response.
Your risk for any STI from this encounter is low. I fully anticipate that your HIV status will remain negative following the encounter youhave described.
This concludes this thread. Take care. EWH
