Hello, Doctors. I would wish to know how much risk am I in considering my exposure.

I engaged the services from a Japanese female escort on 18th March 2024. Prior to the encounter, I clarified that the escort last did a comprehensive check up in late January 2024, and all was clear. During act, we kissed, proceed to unprotected oral, followed by protected vaginal-penile intercourse, and the condom was intact after the act. 

Considering the above situation, may I clarify what are the chances of HIV and other STDs? Is the negative results dated 2 months ago still a relevant factor to consider? Is PEP necessary and when should I get tested (assuming its the Alere 4th gen Ab/Ag rapid finger prick test, offered by clinic nearby)?

To add, I performed cunnilingus on said escort. Should this be a concern too? Thankn you.

Welcome to the Forum and thanks for the questions.  I’ll be glad to comment.  The encounter you describe was close to no risk for HIV and other STIs.  Most commercial sex workers do not have HIV or other STIs and most single exposures do not lead to infection.  Your partner’s recent negative tests are further evidence that she was unlikely to be infected.  In addition your use of a condoms reduces your risk for infection to virtually zero.  

I would not recommend PEP for you.  Your risk is close to zero and taking PEP would mean that you could not test for HIV until after completion ofvPEP, prolonging any uncertainties you might have.   EWH

May I seek your advice as to when should I do a checkup for assurance sake, since information varies online? Espeically testing by the Alere rapid test via finger prick method?

As I said, above, your risk of acquiring any STI, including HIV is very, very low. 

Given you were exposures, you could test for gonorrhea and chlamydia with a urine test and a swab test taken from your throat since you performed cunnilinglus on your partner.  

For testing for HIV you will have to wait to test. Testing using HIVRNAPCR provides conclusive results anytime more than 11 to 12 days following an exposure but no sooner. Testing using a fourth generation combination HIV antigen/antibody test will provide conclusive results at any time more than six weeks following your exposure. Most rapid test test only for antibodies and do not provide concussive results until eight weeks following in exposure. 

If you choose to test for syphilis, syphilis, blood test could be obtained at any time more than six weeks after your exposure. No matter how or when you choose the test. The likelihood that you have been infected is very low. EWH
