[Question #11143] Unprotected oral sex

Hey doctors,
Ive been super stressed and anxious for the past week. Its really impacting my life.
A week ago (25/3) i visitted a massage parlor in Sydney where i live. The female masseuse unexpectedly gave me unprotected oral sex where I came in her mouth, the time window wouldve been less than a mintue. She was young early 20s. Said shes only been in Australia for a month, previous to that she lived in Japan and only started in massage/sex work after moving to Australia a month ago. 3 days after the encounter i started feeling slight pain and discomfort in my penis and scrotum (really hard to put a finger on where the pain is), with slightly higher frequency in urination. No discharge. 7 days later (1/4) im still feeling pain and discomfort in my groin. Ive noticed the symptoms more after urination and while sitting down. Ive got an appointment today to go see the dr to get tested. Im super stressed and anxious because ive never felt this before and reading the internet doesnt help. My greatest fear is 'super gonorrhea' and living with this the rest of my life.

This was a very low risk exposure. Most commercial sex workers do not have STIs and STIs are rarely acquired from receipt of oral sex. When STIs are acquired, it is usually Gonorrhea which is typically an option as asymtomatic with and obvious penile discharge and burning on urination. Urinary frequency is not a symptom of Gonorrhea but is a symptom of anxiety. Your other symptoms are not symptoms of typical STI.
I really would not be worried, but testing might be reassuring. I anticipate that if you choose to test, the results will be conclusive at this time and will prove that you do not have gonorrhea or STI.

Thanks for the quick and prompt response Dr Hook.
In that case what could be the cause of my symptoms? These symptoms started 3 days after my encounter, have been experiencing them since last Thursday. Is it possible UTI or NGU? This discomfort and pain is lingering. Is there anything i should ask the dr to check up on?

Your symptoms are more consistent with anxiety and heightened awareness of normal genital sensations as you have focused on evaluating your symptoms . Theyy really don’t suggest an STI. Please let us know what they say following your examination tomorrow.

Hey Dr Hook,
Blood tests came back all negative for hiv, syph. Doctor said to check again in 3 months as syph takes time to incubate.
And both urine and urethral swab came back negative for gonn, chlam, and UTI. No bacteria or WBCs detected. However when the doctor did the urethral swab, it was really painful for me.
Im super relieved theres no STI. To be safe i plan to go back in a month and do the tests again. What could possibly be causing the sensation and discomfort im feeling in my testacles and penis?
Today is day 10 post encounter and im still feeling an unusual sensation (increased after urination) and felt more when sitting down, not pain anymore but i definitely dont feel like my usual self. Its at a 3/10 on the pain scale at the moment. Is there any further tests i should be doing to rule out anything serious?
Im trying to think of some possible explanations:
- the day before and after exposure i was eating spicy food which caused some diarrhoea. Maybe the spice from the food inflamed irritated my urinary tract?
- I went to see a chiropractor the day after exposure, could these sensations be referred pain coming from my back?
- or the masseuse was really rough when she was giving oral and what im experiencing is soreness post event. But the symptoms started 3 days post and im still feeling it 10days post.
Would appreciate any recommendations on next steps. Should i be seeing a sexual health specialist or urologist?

Adding more info: went to get a 2nd opinion and 2nd urine test done to be completely sure. The dr said it could be prostatitis and prescribed me Trimethoprim. I have not started taking it yet, given my symptoms would be a good idea to start them despite the negative urine tests.

I agree with you that the accumulating evidences that there is no STI present. Overall, your risk of syphilis is very, very low. As to what might be causing this it is more difficult to say. As I have already mentioned often, we find the clients who are anxious and looking for symptoms, tend to experience symptoms, quite similar to yours. I would not be too worried that the intensity of the exposure led to this irritation. Had that been the case, it should’ve resolved by day 10.
Back pain can be referred to the scrotum, but is rather unlikely.
I am hesitant to call this prostatitis, unless there is evidence of inflammation, or a doctor has examined you not simply guess that this is a possibility. If you were concerned about prostatitis, you should be seen by a urologist. I would wait to start the trimethoprim until you have more information.
