[Question #11148] Can I move along?

I am a male age 49.  I do have hpv.  It flares up every few years.

I recently hooked up w/ an oriental girl.  We both live in the US.  We kissed. I played with her breasts and she provided unprotected oral sex.  I also fingered her with a minor finger wound.  After this event my abdomen started to hurt as did my testicle.  My doctor called in antibiotics which eliminated the problem.

I asked the girl to get hiv tested.  She was negative about two weeks after this event.  She had an alere rapid duo performed.  She did mention she takes supplemental biotin (20 to 30 mcg daily). She said her last risk was during the summer, but I dont know if I can trust her.

Since this event I have NOT had any fevers, sore throat or rash.

My symptoms were what appeared to be one swollen lymph node found by my doctor.  He also found a lump he wants to investigate just to be safe.  This was at around 7 to 8 weeks post exposure.  At 9 weeks post exposure i had some body aches.

My testing: Alere rapid duos at 3 weeks, 4 weeks, 6 weeks, 8 weeks and 10 weeks.  All were negative.  I also had two rapid antibody tests on day 64 and 66.  Both were negative.

I also tested negative for HCV at 9 weeks post risk.

Something seems to be wrong with the website.  I am sorry.   My questions are below.

1. Did I rule out HIV 1 & 2?
2. Did the amount of supplemental biotin along with anything in her diet affect her test results?
3. When are hiv antibody tests conclusive for both hiv 1 & 2? ( I saw posts with different answers)
4. I know hiv 2 is ruled out only via antibody tests.  When can this be conclusively ruled out?
5.  I have returned to my wife after years of seperation. Can I sleep with her unprotected at this time?
6. Did my testing rule out that I had ARS?
7. Do I need more hiv tests at all or is 10 weeks good enough?

Welcome to the forum. Thank you for your question and your confidence in our services.

Even if your partner had HIV, the events you describe could not have infected you with HIV. There has never been a proved case of HIV transmitted mouth to penis and therefore no need for you to be tested, or for you to ask that she be tested. And a single lymph node after 8 weeks not only cannot be HIV given your negative test results -- it undoubtedly has nothing to do with sexual event described. The risk of other STDs was slightly higher, but still extremely low.

1,7. "Did I rule out HIV 1 & 2?" Yes, with giant overkill! You didn't need all those tests and certainly shouldn't have any more!
2. Biotin? The online business about biotin and HIV testing is way overblown. The doses she took did not interfere with her test results.
3,4. The AgAb ("4th generation", "duo") tests are conclusive after 6 weeks. That includes Alere, and the results are conclusive for both HIV1 and HIV2.
5. From an HIV standpoint, there is no reason not to resume unprotected sex with your wife.
6. Yes, your negative test results rule out ARS. It is impossible to have ARS symptoms and test negative for HIV.

You misunderstand sexually transmitted HCV. It does not occur from heterosexual sex at all, and certainly not from the kinds of contact described. HCV is sexually transmitted only between men having potentially traumatic anal sex with other men.

Receiving unprotected oral sex carried possible risk of gonorrhea and syphilis. Both are very rare without symptoms, but far more common than HIV. You might want to consider a urine gonorrhea test and syphilis blood test for reassurance.

I hope these comments are helpful. Let me know if anything isn't clear.

Dr. Handsfield,

Thank you for your response.  As I mentioned earlier my risk was unprotected oral and fingering with a nick on my finger.

I spoke with my doctor and he said the antibiotics I was given should have aborted any std.

Since I posted my question yesterday I had yet another Alere rapid duo at 73 days post risk.  It was negative.

1. Is fingering a vagina with a little nick a risk for hiv?

2.  I have read on the forum hiv 2 is ruled out at 6 weeks and sometimes 8 weeks.  My question is did my 10 week (73 day rapid duo test) conclusively rule out hiv2?

3.  When are hiv antibody (only) tests deemed conclusive?

4.  Can I return to my wife sexually with no fears ?

5.  Can I stop hiv testing?

6.  Is the real answer I need counseling to move forward?

The service you provide is truly helpful. I just want to move on. Thank you.

You don't say what antibiotics your doctor prescribed, but they weren't necessary; no STD was likely from the exposure described and no STD was a possible cause of the symptoms you had at the time. I'm confident the antibiotic made no difference; your symptoms were going to clear up anyway.

These questions already were answered above, or the answers are obvious from my replies yesterday.

1. Zero risk. Nobody ever gets HIV from fingering, with or without finger nicks, sores, etc.
2. See yesterday's response (1,7). I see no reason to repeat myself!
3. The newer standalone HIV antibody tests ("third generation") are conclusive by 8 weeks.
4. You never had to stop having sex with your wife. You certainly can resume sex now with no worries about giving her HIV.
5. See above.
6. Whenever someone suggests his own problem has a psychological origin and might benefit from counseling, almost always he is correct. You should consider it.

In the meantime, I would advise that you obviously regret and have anxieties about the hook-up you described above. Deal with those issues as you need to to, perhaps with counseling -- but don't confuse them with HIV/STI risk. They aren't the same and the events you describe were entirely free of HIV/STI risk.

Thanks for the thanks. I hope these additional replies do help you to move on. Good luck.

Dr. Handsfield,

Because I had a rapid Alere 4th gen test scheduled I had it done on Friday.  It was negative.  This was 75 days post risk.

Questions:

19 hours prior to this test I had a smoothie which contained 1 banana, some blueberries and some peanut butter.  I know those things contain biotin.  But I also read something from Dr Hook that said after 8 hrs biotin intake would be negligible.

1.  Did this smoothie affect my results?  I am worried about the biotin.

2. Did this test conclusively rule out hiv2?

3. Can I finally sleep with my wife?

I am sorry I am having a hard time moving on.

1. Biotin in the amounts in foods and standard vitamin supplements has no effect at all onHIV testing. The smoothie did not affect your results. You could have been tested accurately even a few minutes after the smoothie or following a vitamin containing biotin.

2. Yes, it ruled out HIV2.

3. There was never any need to stop sleeping with your wife. For sure you should resume sex with her now.

That concludes this thread. I hope the discussion has been helpful. Best wishes and stay safe.
