[Question #11166] hiv risk and testing timeline

I had an act with a stanger (m2m) in sauna. we frennch kissed with his toung steering in my mouth. He strock his penis and rubed his precum on to my penis and strocked mine a few times. He tried to sit on my croch but i pushed him away. Then he rubed his precum on to his nipples which i did not notice at that time but I sucked and licked it. Then I kissd his cheek and felt pain on my lip from the sting by his hair. Later i found out that my mouth has wounds and huge uclers. I ask him to take a test, he refused and i got scared and started to take FTC+TAF+DTG.
I consulted with an infectious doctor and he said there is no risk, so I stopped it on day 3. Then visitd a CDC doctor and he metioned there is a little risk. and pep is stopped on day 3 can only take a 3rd gen test on 90th day by guideline.
1. could you assess if i had risk from those activities? if the wound on my lip contact the precum and the wound got poked by hair, will that create higher risk?
2. since 3 days of PEP was taken, when can i take a conclusive test?

Welcome to our Forum. Thanks for your questions. I'll be glad to comment.
The exposure that you describe was very low risk for HIV. I'm sorry to hear that your partner refused to test- I understand why that can be concerning. At the same time however, this was a no risk event for HIV. HIV is not transmitted by kissing, even vigorous French kissing. In addition contact with a partner's genital secretions without penetration is a no risk event as well. HIV is Sexually transmitted ONLY through penetrative sexual contact. Therefore when partner's get each other's genital secretions on one another, even if the contact is near the urethra. There is no evidence that swallowing genital secretions without penetration (for instance if you were kissing parts of his body which had genital secretions on them) would put you at risk for HIV. If you had asked me if you needed PEP, I would have said no. I'm glad you stopped taking the medication. If you want to test (I do not see the need) testing 6 weeks after your last dose of medication will provide conclusive results which will prove that you were not infected.
Your mouth sores are not due to HIV and do not increase your risk for HIV

Hi Dr Hook, thanks for commenting
And thanks for the postive opinion on the early PEP termination, I was so paniced when the doctor told me there was a little risk and PEP was a choice could be made. Have a few follow up questions.
1. Seems that my experince is The exposure was very low risk for HIV but no risk event for HIV. how to understand this? is it like theorically possible, but pratically it doesnt happen to this kind of event?
2. In the picture shown, the wound on the outer lip and ucler are both broken, if it is opening when covered with precum. broken ucler and broken wound on outer lip will this detail affect the risk?
3. You metioend no risk when genital secretions contact is near the urethra, if some got on the urethra is it risky?
4. Is it recommended to take 4th gen ab/ag test or 3rd gen ab test?

In medicine strange things happen and we learn new things continuously. Therefore we never say "never". On the other hand, your risk for HIV from the activities you describe is virtually zero.
2. In the picture shown, the wound on the outer lip and ucler are both broken, if it is opening when covered with precum. broken ucler and broken wound on outer lip will this detail affect the risk?
We do not view pictures on this site. Past experience has proven to be unreliable and often misleading, sorry. Surface contamination of open sores, ulcers, scrapes ais not an increased risk factor for acquistion of HIV.
3. You metioend no risk when genital secretions contact is near the urethra, if some got on the urethra is it risky?
No
4. Is it recommended to take 4th gen ab/ag test or 3rd gen ab test?
If you chose to test, a 4th generation, combination HIV antigen/antibody test is preferable. They provide conclusive results sooner (at 6 weeks vs 8 weeks for 3rd generation, antibody only tests)
I hope this information is helpful. Please don't worry. EWH

Hello Dr Hook
Thank you for the for the kind words. Your detailed and professinal advises really relieve me from the anxity. I will take a 4th gen test after 2 weeks(6 weeks after last does of pep) for assurance. And I have a good feeling about it.
Thank you so much for your time
