I met a female sex worker today. We kissed, and she rode on my face with her panties on. I licked her genital area, anus, and rectum (with her panties on), and I clearly felt that her genital fluids entered my mouth and made my entire face wet.

She then took off her panties, immediately inserted them firmly into my mouth, and then she gave me a handjob.

My questions are:

1. Can HIV be transmitted through panties, bras, socks, stockings, and other clothes that are wet with active HIV-infected genital fluids and blood, for example, through contact with my anus, rectum, and penis?

2. When she was riding on my face, I felt that my face was wet. Was I at risk of being infected with HIV if her genital fluids entered my nose and eyes?

3. Imagine a situation where a sex worker who is HIV-positive takes off her panties that are full of her active genital fluids and immediately puts them on my penis, using them to stroke my penis firmly. Are there any HIV risks in this situation?

4. Can I move on with my life without HIV testing?

Welcome to our forum. Thank you for your questions. I’ll be glad to comment. The encounter you describe was virtually no risk for acquisition of HIV.  Before I address your specific questions let me point out that even most commercial sex workers do not have untreated HIV. Thus, the likelihood that your partner had HIV to transmit is low. I’ll answer your specific questions below.

1.  HIV is not transmitted through clothing. Even clothing that is wet with genital secretions. The virus becomes inactive upon exposure to the environment outside of the body and has a tendency to stick to the fibers of cloth and clothing.

2.  This too was no risk, even swallowing genital secretions would not put you at risk for acquisition of HIV. There must be direct contact and friction is needed for transmission. While not relevant to your situation, this is the reason that contaminated food does not transmit HIV either.

3.  No, this is no risk also for all of the reasons that I have already stated.

4.  HIV testing is always a personal decision however, if I were in your situation, I would feel no need to test for HIV.

Thank you for your reply, which is really helpful! I have some follow-up questions:

1. Is my risk of HIV transmission zero from her inserting the panties she had just removed, which had her fresh genital fluids, into my mouth? The panties remained in my mouth for over half an hour throughout her performance of the handjob on me.

2. When you mention that direct contact and friction are necessary for transmission, what exactly does that entail? When she sat on my face, my mouth and tongue were indeed in direct contact with, and rubbing against, her genital area, anus, and rectum (while she was wearing panties), and there were abundant genital fluids. Can you confirm if my risk from this exposure is indeed zero?

1.  Yes. This is still zero risk.
2.  Penetrative sexual contact, I.e. Insertion of the penis into the vagina, rectum, or mouth is required for HIV transmission.
