[Question #11266] Exposures overlap and Pep concern ( "late Pep")

Good day kind experts. Greetings from South Africa. Gay male. Hiv 1 & 2 concern and using Pep and overlapping exposures. My first exposure happend on the October-22-2023 tested negative for Hiv1 & Hiv2 via 4th generation lab on day 21 & day 27 & day 34 post first exposure and immediately placed on Pep on the same day Nov-26-2023 for second exposure ( 16 doses of Pep and discontinued) I had some symptoms before Pep....some during Pep and some sort of viral syndrome after Pep was discontinued. I felt quite ill and still don't feel my current self wich concerns me. I tested negative on 33 post Pep and day 60 post Pep via 4th generation lab negative for Hiv1 & Hiv2 and also just recently had 2 rapid finger prick quick tests for Hiv1 & Hiv2 antibodies on day 116 & day 124 post Pep with negative results. I still have concerns regarding symptoms etc. If i was exposed to Hiv1 or Hiv2 on my first exposure Oct-22-2023 ( Pep started 35 days after) could the 16 doses of Pep have any affect on my current test results if I was in the acute phase of Hiv1 or Hiv2? Could 16 doses of Pep cause delayed seroconversion to Hiv1 or Hiv2 antibodies and possibly affect my later test results and current ones. I had some severe and worrisome symptoms. I have seen 3 Doctors regarding the above mentioned and does not seem concerned. I saw an infectious disease specialist and he stated that if I was truly exposed on my first exposure Oct-22-2023 regardless of the 16 doses of Pep I would still test positive after Pep and current results as Hiv1 & Hiv2 antibodies will still continue to replicate regardless of Pep. Some input would be greatly appreciated.

You asked the same questions twice so I deleted one and have asked that your posting fee be returned. After this response you may ask two follow-ups related to these questions if my answers are not sufficient.
The PEP that you took beginning on Nov 26 would have no impact on your risk for HIV infection from the encounter on October 22. If you were going to be infected, infection would have already been established by Nov 26. Your negative tests subsequently, during a time when you had had, or were having symptoms, would have been positive. The fact that tests 60, 116, and 124 days after PEP certainly prove that you were not infected by the encounters you have provided dates for. You can be confident of this. When symptoms are due to HIV, tests are always positive and remain positive from then onward.
The symptoms you have experienced are due to something other than HIV acquired at the times you describe. Related to those encounters, there is no reason for further testing or concern. It is time to move forward. EWH

Greetings Dr. Edward. I honestly appreciate the feedback and response to my current concerns regarding my over- lapping exposures and Pep. Today I added a negative finger prick rapid antibody test for Hiv 1 & Hiv2 wich is 138 days post Pep. So my concern for delayed seroconversion due to Pep ( 16 doses) and overlapping exposures isn't a concern at all? And that if I was truly exposed to Hiv1 or Hiv 2 on my 1st exposure Oct-22-2023 and then started Pep for my second exposure Nov-26-2023 it will not have any affect on my current results for these exposures? And if my symptoms were indeed Hiv1 or Hiv2 related I would test antibody positive at this stage 138 days post Pep. This has been such an exhausting and fear mongering couple of months feeling the way I do and all the symptoms etc. I will never put myself at risk like this again in my life! Lastly...just for interest sake....could any underlying conditions...illnesses or disease affect the liability of Hiv test results? Can I move forward?

There are no concerns for delayed seroconversion and, as I told you, your test results absolutely PROVE that any symptoms you experienced before or after testing were NOT due to HIV-1 or HIV-2.
I encourage you to move forward. You have proved you were not infected. EWH

Thank you Dr Edward. I will move forward and keep working with my Doctor to find the possible cause of the symptoms I had as it's not related to Hiv1 or Hiv2. I appreciate the response. It brought some light and calmness to my situation and concern.
