[Question #11302] HIV Risk

Good evening. I had an encounter with a random girl I met in a nightclub in Guadalajara, Jalisco, Mexico, two weeks ago. She asked me to wear a condom, and I did, but sometime during sex I’m not pretty sure how it happened because I was drunk and was not wearing the condom anymore. I found it near me, and it had some blood of the girl, and I had a little blood on my penis. I did not ejaculate, and we stopped. I contacted her a week later to ask her if she had performed any tests in the past, and she informed me the last one she had was in January, and all was ok, but she broke up with her boyfriend because he cheated on her last month. We only had vaginal sex. Anxiety is eating me alive. I am a 23 Years Hispanic uncircumcised  
 She is 22. My questions are: 
Was this a high-risk exposure? 
Can I take the HIV RNA test on day 15 after exposure, or is it a waste of money? 
Are std check.com tests reliable?
It looks like that girl parties a lot and I am so worried I haven't been able to sleep well. Thank you so much

Welcome to our Forum. Thanks for your questions.  I'll be glad to comment.  The encounter you describe was low risk.  I say this because most women do not have STIs, including HIV and most single unprtected expsoures do not lead to infection.  That you were exposed to here blood makes no difference- blood is no more infectious than vaginal secretions.

In response to your specific questions:
Was this a high-risk exposure? 
See above

Can I take the HIV RNA test on day 15 after exposure, or is it a waste of money? 
HIV PCR tests are highly reliable and provide virtually conclusive evidence regarding the presence or absence of infection when taken any time more than 11-12 days after an expsoure.  As you point out however, they are expensive.  FYI, you can test for other STIs (gonorrhea and chlamydia) with a urine test any time more than 3-4 days after an exposure.

Are std check.com tests reliable?
Yes, but they too are rather expensive and sometimes test for things that do not need testing for.  Less expensive, targeted testing is typically available at health departments.

Hi Doctor, good evening. I took the test on the 15th day after the exposure, and it returned negative. Do I need to retest for HIV, or is this test reliable enough? 

Also, If I have to retest, what day can I take the test to make it conclusive? 

The other two tests you mentioned in past questions are already 100 conclusive?

I would have confidence in this result.  I see no need to retest for anything.  Time to put your concerns aside and move forward.  EWH---

Hi, I made a mistake. I had sex with my girlfriend, and although I used a condom during the whole vaginal sex correctly, she performed unprotected oral sex on me. I have not taken the tests yet to check for Syphilis, Gonorrhea, or Chlamydia. The issue is that I noticed that I have red pimples on my penis that weren't there before. They look like HSV 2, but they also look more like balanitis. After searching online, I am not sure. After the sex, I felt very itchy on my penis, and after checking it in, I noticed those red pimples were bright red and more noticeable.

-If it were the case that this is HSV 2, can I transmit it to my girlfriend through oral sex? I read that it is a Genital disease, but I don't have it clear.
-In case this is clhaymidya, syphilis, or Gonorrhea, was this oral sex a high-risk exposure for her?
-Should I take her to get tested with me?
-Syphilis Chancroid can be confused with the symptoms that I just described or the Syphilis symptom is more obvious? 
-Syphylis Chancroid only appears for the zone that the bacteria enter to the body or it can appear in a different zone? I mean like, if I had vaginal sex and I got infected trough my penis can the chancroid appear on the anus?

I'll go straight to your final follow ups.  The "pimples" you have noted are almost certainly not herpes- if you were going to develop herpes lesions, they would have appeared within 10 days of your exposure and it is now nearly 20 days since, ruling out herpes.  Further, if the bumps were herpes, they would have evolved from bumps to small blisters and then small open ulcers within a day or so of appearance.  These bumps are NOT herpes acquired from the encounter you described.  Similarly, what you describe sounds nothing like gonorrhea, syphilis, chlamydia, or chancroid.  The bumps are probably totally unrelated to the expsoure you describe.  Thus:-

This is NOT HSV-2, see above.  You need to be examined by a knowledgeable clinician who can evaluate and order the tests that that you need

It sounds like the bumps appeared after she performed oral sex on you.  They may be from her.  If she is feeling poorly, she should be examined along with you

See above.  If you think it is related to your encounter with her, she should be examined along with you

-Syphilis Chancroid can be confused with the symptoms that I just described or the Syphilis symptom is more obvious? 
This sounds nothing like any of these problems

If the bumps were any of these diseases (I am confident they are not) they would occur ONLY at the sites of direct contact, not elsewhere in your body
