[Question #11319] Confused if i have HPV or not.

Hello doctor. Hope this finds you well.

So back in march 2022. I had sex protected. But, im not sure if the condom broke or not. 

However, fast forward to June 2023. I noticed a white flat spot on my shaft. Then i went to a dermatologist and he diagnosed me with Milia. Then, i squeezed it (stupid move) then slightly got bigger and bigger. So i left it. After some time on February 2024. I decide to go another dermatology. He really wasn’t sure what it was. He said the bump was formed in strange way. 

However, he said give it more time, then he gave me supplements, [Zinc & B12] and with using the supplements for 3 weeks the bump started to scab and healed.it didn’t fall off or anything. It just scabbed and healed. It left white small patch/dot. 

I went back to the dermatologist and showed him the stages of the bump.

He said its not HPV. He said HPV only scab by using treatments like creams and cyro. As for my bump scabbed without using any treatments. 
He continues saying that HPV warts just fall off or decrease in size and disappears. And my case it just scabbed. And he diagnosed me with dermatitis.

Since last June 2023, i only had 1 bump, never developed another one, even after it scabbed it went away 3 months ago, never developed any bump besides the first one im talking about. 

I don’t know if the dermatologist words are right, but, do i have an HPV? Or he is right?

Welcome to the forum. Most users don't get nearly real-time replies -- I happened to log in immediately after you posted your question.

In principle, almost anyone can have HPV, which is a normal, expected, unavoidable aspect of being sexual. Around 90% of all sexually active persons are infected at one time or another. Most infections never are apparent:  no warts or other symptoms, no known health impact.

However, almost certainly the penile problem you describe is not a wart or other sort of HPV. First, although the appearance probably was altered by the various manipulations you describe, its initial appearance did not suggest a wart; and no dermatologist is likely to confuse a wart with milia (a normal skin condition). And although your dermatologist probably is more expert than I am, I agree with your description of his advice about warts versus other kinds of lesions. Further, genital warts rarely appear as single lesions:  usually there are several. In addition, over 95% of genital skin problems are not STDs of any kind:  don't assume HPV or other STI just because you have genital bumps, rashes, etc.

You really needn't be at all worried. Of course play it safe and return to the dermatologist if the problem persists, if new bumps appear, or you otherwise remain concerned.

I forgot to include our standard advice about being immunized against HPV. All sexually active young persons, or potentially sexually active in the future, should be vaccinated. The vaccine is 100% protective against the 9 types of HPV that cause 90% of genital warts and various cancers. The risk for those things is fairly low, but still everyone should be protected. Discuss it with your personal physician.---

Correct, we do not examine clinical photos. We are not a substitute for direct medical care and examining images comes too close to practicing medicine. In any case, we re not nearly as reliable examiners of genital skin lesions as a dermatologist is.

So, i can be sure that i don’t have HPV, because Genital warts don’t scab by their own. Right?

Correct:  genital warts don't scab except in response to inflammatory treatment. When warts spontaneously resolve, they simply melt away and are gone.
