Hi Drs.
Does the duration of oral make any difference? I.e. unprotected insertive for 2 hours with a high end CSW? Would you advise testing?

Logically, longer duration exposure might increase the risk of STI transmission compared with shorter events, but there are no data on this. Over the years, almost all forum questions on this issue have asked about brief or fleeting exposures, like only a few seconds -- in comparison with more typical exposures of perhaps a few minutes. This the first I recall about a two hour event!
Whether your partner had an STI transmittable by oral sex is more important consideration than duration of exposure. In that regard, "high end escort" implies a low chance: most such female sex workers know the score, take precautions like consistent condom use for vaginal or anal sex, get tested frequently, and have clients who are mostly low risk themselves (men like you). So the odds are strong that you were at little or no risk. However, since you're nervous enough about it to even ask the question, you might consider testing strictly for reassurance. If so, you could have a throat swab for gonorrhea and chlamydia and a blood tests for syphilis and HIV in a few weeks. No other useful testing is available in absence of symptoms like penile discharge or sores.

You’re correct in that she is/ was precautious and did tell me she was clean. Would you be worried about HIV in this scenario or does the guidance still stand that in all likelihood this is not a risk event (barring any anxieties, etc. one may have); in other words, should I be worried about engaging in intercourse with others?

Zero risk for HIV -- both because your partner is extremely unlikely to be infected and there has never been a proved case of HIV transmitted oral to genital. You definitely should not seek PEP, nor worried about continuing sex with other partners on account of HIV. The risk for other partners is higher for other STDs, but low enough to disregard as long as you don't develop penile symptoms in the 10 days or so after the event.---

And sorry for the dumb follow up- I wasn’t sure if the duration counted as high risk given you and Dr. Hook always say ‘you won’t be the first and when it will be it will be high risk’
