[Question #11400] Urithritis? STI? UTI?

I took a weekend trip with some friends and ended up having 3 sexual encounters, all 3 were 100% protected with no doubts. With 2 of them, it felt a bit dry and rougher, but I paid no mind. Another note is that the soap provided at the airbnb left my skin feeling like sand. The last interaction took place on Saturday and upon arriving in Miami on Monday afternoon I immediately went to urgent care to have a panel done, no symptoms were present at the moment. That night after taking the test I chose to take a 2G dose of Azithromycin followed by 3 days 500mg. the following days I began feeling a slight itch and made nothing of it. It became a little more prevalent as the days passed. There is no pain during urination, discharge, or discomfort around the pelvic area. The test results did come back negative for chlamydia, Gonorrhea, syphilis, and HIV. Once a little panic set in I decided to get a urine test done (I'm assuming the Azithromycin Is still in my system and won't give an accurate result) and visit a urologist to explain the situation. He explained to me that based solely on the urine test there was nothing to worry about and proceeded to examine me. He does see a small what could be described as a white head on the inside of my urethra and says that could be irritation from friction. He proceeded to prescribe me Rigresan 3G and Fluconazole 150mg as a one-time dose for both, the day after the small white head did disappear but I am still dealing with slightly irritated urethra and now a slight discomfort in my pelvic area. Its been just about 48 hours since taking those single dose medictions.

My questions is, after that hard hit of azithromycin could there still be a possibility of STI?

Could the Azithromycin have affected me (was the dose overkill)

You overreacted to a virtually zero risk situation in regard to STIs, especially any possible urethral infection. I would have advised against the azithromycin treatment and against being tested for any STI or seeing a urologist or any other health care provider. At this point, I have to suspect any symptoms you have are the result of the excessive antibiotic treatment and/or anxiety.

I don't know how to interpret a possible white head in your urethra and have trouble believing any infection was there to treat with either fluconazole or fosfomycin (Rigresan).

The azithromycin would have aborted any newly acquired syphilis or chlamydia, and probably gonorrhea -- but given the condom use, you were not at risk for any of these. Yes that was "overkill"! I don't recommend any further testing. I'm confident any recitation "irritated" feeling in your urethra is minor irritation or anxiety and I would expect it to resolve without any further treatment.

Good afternoon Doctor, my apologies for the delayed response. it has now been 9 days since those last medications and I can say that I am still feeling a slight discomfort. when my bladder begins to fill I can feel a pressure at the head of my penis. not painful but noticeable. the only time I feel no symptoms at all is first thing in the morning before I've had anything to eat or drink.  I'm also now feeling a slight pressure in my pelvic region from time to time as if someone is very gently poking me. I'm a bit worried that the azithromycin dose i took may have done some damage. also aware that anxiety is playing a part but none the less I'm certain there is something there.

No STI causes such symptoms and azithromycin cannot "damage" the urinary tract. These symptoms are typical for genitally focused anxiety, nothing more. I could use your words to teach medical students about it -- no matter how certain you feel about it.---
