[Question #11442] Protected sex

Hello doctors!
I am coming back with a new question.
On wednesday night i had an encounter with an escort oral (she gabe me fellatio) and vaginal both protected with condom  for 10 minutes. Also thursday  I had another episode of oral ( she gave  me fellatio)and vaginal sex protected with condom (  10-12 minutes)with another and diferent escort, and now I am very scared .
Please be so kind and tell me what possible ITS  i could get infected with?( Gono, syphillis , hepatitis B , HIV)
The condom was worm for both encounters and all the time. I even checked them with water after.

Welcome back to the forum. However, I do not understand the need. I reviewed your recent discussion with Dr. Hook. You asked the same things there and Dr. Hook's reply was very clear.

It is impossible to catch any STI from condom-protected oral sex, and the risk with condom-protected vaginal sex is very low. There is almost no chance you could get any of the STIs mentioned. I recommend against any testing at all. (The condom "water test" is unnecessary; despite what you can find online, there is no such thing as microscopic leaks that can allow STI transmission. But in any case, that result also should measure you.)

So all is well, nothing need be done. I hope this information is helpful. In that case, I suggest you re-read your recent discussion with Dr. Hook.

Thank you very much for your  reassuring answer.
The difference compared to what was discussed with Dr. Hook is that then it was only about protected oral sex,it was not vaginal sex but now it is also protected vaginal sex and I know it is more risky. And I want to ask you something else, namely the fact that after I took off the condom, I touched the tip of my penis with my hands and there may have been seminal fluid from the condom on my hands. Can gonorrhea be transmitted in this way? Or other infections? And having a st ate of panic, I took 200 mg of doxycycline (doxy PEP) this morning, 8 hours after sexual contact. Do you think that if it were syphilis, is this enough ?I know that protected sex is safe sex, but I still can't calm down. I'm afraid of HIV even though I know it was protected.( oral and vaginal) what are the chances of getting HIV in this situation?
and the fact that there were 2 different escorts in two different days does not increase the risk?Do you think other STIs such as herpes or HPV are a threat in this situation?The anxiety is high due to guilt and fear and I am trying to calm down and I can do it by reading your answers.

The condom touching etc you describe happens all the time in condom users and nobody gets infected as a result. These issues do not change the risk of catching gonorrhea or any other STI and does not change my overall evaluation and advice. I also understood the two different sex workers.

I'm afraid I really can't help further. That you are "afraid of HIV even though I know it was protected" and "still can't calm down" is your problem to solve, not mine. You have my advice and it will not change. Of course you are free to be tested if the negative results would help you calm down!

Jello again and thank you for your :From a medical point of view, please answer the following questions:
1. Is Doxy PEP 200 mg enough to clear syphilis?
2.In percentages, what are the chances of contracting HIV in my situation?
3. when do the first symptoms of gonorrhea start? Days
4.Is there any possibility of contracting hepatitis B through protected sexual contact?
5 What about genital herpes?

1. Doxy PEP is not advised except in much higher risk situation than this. But yes, 200 mg is highly effective in preventing syphilis and chlamydia, not as good against gonorrhea. (As it happens, at this very moment I am traveling with the top senior scientist behind all doxy PEP research in the US. She agrees!)
1. Under one chance in millions.
3. Typically 2-3 days, sometimes up to 5 days.
4. Zero risk for all practical purposes.
5. Same for this exposure.

One more thing. Friday I took 200 mg of doxy and today -saturday another 200 mg of doxy. Do you think it’s enough.
And i go to take a gonorrhea test on Monday, ( 4 day after last encounter) it's ok, doxycycline does not influence the test result?

The first dose is all you needed. Don’t take any more. There is no point in testing for gonorrhea if you have no symptoms, especially since your exposure was no risk for it.---
