With counseling, my 18-wk HSV-2 western blot and wk 1 to wks 29 IGG HSV-2 all negative, I am beginning to accept the positive HSV 1/2 - IGM at wk 8 was false/reactivation of long-term + HSV-1. My experience was a one-time hand job where the female spit directly on my penis. I am not sure if any salvia entered my urethra. I never had discharge, blisters or lesions nor did I take antiviral meds. Remaining concerns/symptoms of irritation or sensation when my penis touches my underwear/clothing mostly while sitting or walking light urethra discomfort.
Is this a symptom of HSV? If no, any other STI this can be associated with?
What STI’s was I exposed to in this situation?

I'm so sorry that someone made the error of ordering an IgM test for you. In my hands, they are wrong 85% of the time, and big labs, like they Mayo Clinic, stopped using them entirely in 2019.
Trust the western blot completely.
Your hand job present close to zero risk for HSV 2 and the irritation is due to something else entirely.
I don't believe that you were exposed to any STI's in your situation - did you get checked for routine STI's like gonorrhea and chlamydia?

Yes, i was tested for those and more wks 1, 3, 8, and the many 10 panel tests taken, and all were negative. They didn't give me any meds because i was always negative but in pain. I only got antibiotics in late January from the 2nd urologist. I wonder if it missed something, and it later created a major problem. Everything has been negative except the HSV-1 and HSV1/2 IGM. My main concern is making sure its not HSV-2.
When the IGM is correct, by week 18 the body would have made IGG antibodies to be detected by the blot?
Hopefully, i will get to the bottom of this remaining pain.

Have you taken antiviral medication, like Valtrex or acyclovir? If yes, for how long? I ask because they can interfere with antibody development.
If you've not taken long courses of antiviral medication, then yes, if the IgM was correct, the western blot would have been positive by week 18

No ma'am. No antivirals zero/none. Only antibiotics started them in late January and now on alpha blockers.

So in that case, believe the negative western blot. And completely ignore the IgM test - it is deeply flawed which is why so many big labs have discontinued using it.
