[Question #11470] Unprotected oral

I am a male in a long term relationship. Seven weeks ago I had an erotic massage with a female sw. No intercourse, but we had mutual unprotected oral. I have been very anxious and went for testing yesterday. I did an hiv rapid test that was negative. They also did throat swab and urine samples. I am awaiting results. I was also given a rapid syphilis test which produced a faint line reactive . About a half hour later they gave me another syphilis rapid test and it was negative. They are testing it in my blood as well. I’m very anxious. Why would the test first show positive and then negative? I have not had any signs of syphilis . Some testicle and a little discomfort in penis. What are my risks? The rest of my tests will not be back until next week. Please give me insight? Thank you.

Welcome to the Forum. Thanks for your questions. I'll be glad to comment. Your risk for HIV from the encounter you describe is close to zero. There has never been a case of HIV proven to have been acquired from receipt of oral sex and the CDC estimates that the risk of performing oral sex on an untreated HIV infected person is less than 1 in 10,000 and that estimate is likely conservative. When you combine these facts with the fact that it is unlikely that your partner had HIV, your risk is effective zero.
There is likewise a miniscule risk for acquiring gonorrhea from the encounter you describe and a still lower risk for chlamydia. I anticipate that your tests will be negative and when they are, you can move on with no concerns- the results will be conclusive.
Rapid tests for syphilis tend to be finicky and I suspect the faint line you saw on the first test syphilis test was incorrect. Your blood test will provide more reliable results and I anticipate that this too will be negative.
Bottom line, the encounter you describe was very low risk and when your tests return as negative I would encourage you to accept them and move forward. Once your tests are negative, there is no reason for additional testing. EWH

Thanks so much for the response. I am still a bit confused on the syphilis risk and tests . Would I be a high risk for syphilis with the unprotected oral .? Having a difficult time understanding why I had two different results ?

Syphilis is relatively rare among heterosexual women, including CSWs and very rarely transmitted by oral sex. Your risk for syphilis is very, very low.
Your initial test was likely falsely positive- as I said in my original reply- "Rapid tests for syphilis tend to be finicky and I suspect the faint line you saw on the first test syphilis test was incorrect. Your blood test will provide more reliable results and I anticipate that this too will be negative."
