Hello dear gentlemen !
I am coming back to you to ask some more questions to know in the future. please do not consider them repetitive questions.
1.For syphilis, I would like to ask how safe the condom is for oral and vaginal sex (percentage if possible). , and what is the possibility of infection? and the symptoms and what is the onset period?
2.How effective is doxy pep? 200 mg after a maximum of 10 hours (percentage if possible) and assuming that the partner is infected?
3.how long after exposure does genital herpes appear and what are the symptoms?
4.Does the fact that I have oral HSv 1 since childhood influence in any way? So can it be harder or easier to get HSV 2 if  the girl partner ( CSW) gives me fellatio?
5.Gonorrhea and chlamydia test done 4 days after exposure (PCR/DNA test). Negatives. Is it 100% conclusive?
6.The most important: HIV. I read that protected sex is HIV safe sex. But still there is a small chance. What is this?Just like you said in other discussions? The same possibility as being hit by a meteorite?
7.How conclusive are the tests for HIV and Sigilis at 3 weeks and at 4 weeks?
8.How safe is it in my situation to continue unprotected sex with my wife?

A correction to point 2. I took 200 mg of doxycycline  10 hours after the potential exposure and another 200 mg of doxycycline 36 hours after the potential exposure.

One more aspect. At point 4, I wanted to ask the fact that if I have oral herpes since childhood, how does this affect the risk of contracting genital herpes from CSW through protected sexual contact? I think it's clearer now. Thanks for everything!!!

Welcome back, but I'm sorry you felt the need; and some of the answers are obvious from your two previous discussions. 

1. Extremely safe, near zero risk.

2. Repeat question from last time. Doxy-PEP is not advised after exposures like you describe. The research studies show ~80% protection against syphilis, but the true protection probably is 100% when the drug is taken as directed, i.e. within 72 hours of exposure. Some research participants did not take it properly.

3. Symptom once typically 3-5 days, rarely up to 3 weeks. The symptoms themselves are far too extensive and detailed for further response here. Many online resources have such information, including the sponsor of this forum:  www.ashasexualhealth.com.

4. People with prior HSV1 have a somewhat reduced risk of HSV2 if exposed, but you cannot rely on it for good protection. Many people with HSV1 still get genital HSV2.

5.  Yes, conclusive.

6. There is no simple or single answer. Biologically, condoms are nearly perfect, but people make mistakes, condoms break, etc. The risk of HIV from condom protected sex therefore depends on consistent and careful use, good luck (no breakage), and the risk level of one's partners. Overall, the risk surely is higher than a meteorite strike -- maybe more like a lightning strike (?). If you have regular sex with non-committed partners, you should be tested for HIV from time to time even if there have been no apparent condom failures. But not after every exposure. 

7. Probably 75-80% for most syphilis tests, 95% for the most common (antigen-antibody, "4th generation") HIV blood tests.

8. Very safe but there are no 100% guarantees, for the reasons discussed above.

At the risk of repeating myself, I will bother you with the following questions:
1.It is the 9 day since the possible exposure and I have no sign of herpes. I hope it stays like this in the future, because I know that the tests are not recommended. any opinion? Do you think that I have passed the critical period or should I wait until after 10 days as I saw that Mr. Hook recommends?
2.For syphilis, I took 200 mg 10 hours after exposure and another 200 mg 36 hours after exposure. Do you think I should get tested or wait to see if any symptoms appear? Or simply move on…
3.What I read about HIV worries me a lot. Do you think that in my situation I should get tested?  I repeat myself, I know, but I need expert advice and you and de. Hook are the only people in the world at this time who can make me understand and calm me down. Or to continue with my life….I repeat, there were 2 different escorta in two different evenings (everything protected vaginally and orally).
4.Do you think hepatitis B is a threat considering that the two escorts were between 20 and 25 years old and probably vaccinated against hepatitis B. In our country, children are vaccinated against HBV at birth. Is there any point in testing myself?
I'm in a very complicated situation because I'm going on a holiday abroad with my wife and I don't know what to do??!?? Can I continue my sexual life or not, because protected sex with my wife is out of the question.…..

1. The risk of herpes from any single random unprotected sexual exposure is well under one chance in many thousand. Those odds combined with condom use and 9 days without symptoms, you should not be at all worried about it.

2. See previous discussions where you were advised there is no point in testing for syphilis. I haven't changed my mind.

3. Repetitive I have no additional advice about HIV testing than stated in my reply above.

4. There is no meaningful risk of HBV in your situation, also discussed in previous threads.

I can't help that you "don't know what to do", but a forum like this cannot guide you more than we have. 

If you ask anything more that has already been addressed in this or your other two threads, this discussion will immediately be closed without further reply.

Dear doctor,
It was never my intention to cause trouble, but the people who come and ask questions on this forum are mostly anxious and repeat the same questions many times, maybe to be convinced, to reduce their fear by receiving reassuring answers. Once again, I apologize.

You don't have to educate us about the characteristics of forum users and their anxieties! That doesn't give users permission to ask us to spend our time and expertise repeating ourselves.

There is no significant risk of any STI from the two exposures you have described. 

The forum does not permit repeated questions on the same topic or exposure. This being your third, all concerning your inflated anxieties about STI/HIV risks in general and obviously low risk exposures it will have to be your last one; we are not a psychological support service.
