Dear Doctors,I apologize, I had to return here for additional clarification and advise, especially after the assurances given in my previous question. This is because, there are some latest development in the situation based on a dinner I had with her last week, for which your advise would greatly help. I appreciate the great work done here by the experts, where I don't receive the same in any clinics or labs we visit.Just to recall, on Monday, 27 May 2024 (12.30PM) I met an old friend of mine. We were naked and had body to body hug and sleep for sometime. My genital was in contact with her genitals. There were no penetration, while on the bed. I topped her with rubbing of my penis on her vagina exterior. After the session we went to shower together. In the shower (while standing), we hugged each other. She held my penis (without condom and I am not circumcised) and pulled it towards her vagina. My penis tip (Urethra opening) did touch her vagina opening, clitoris.When I met her for dinner last week (Thursday, 13 June 2024), she said she will do a test and she did the HIV Ag/Ab Test (CMIA) on Friday, 14 June 2024. The test came out as Non Reactive. This test was done 18 days after our contact. I also asked her if there was any penetration out of fear, and she said, my penis head/tip did touch the opening of her vagina and she was rubbing on the same, but as she attempted to put my penis in, I pushed her off.  Since 2024, her only sexual relationships has been with her husband and she doesnt know his status. The test taken on Friday, 14 June 2024 (18 days after our relationship) is negative.My question is:Given the latest understanding of how my penis came into contact with her vagina and given her 18 days post exposure test is negative, are there any risks that I need to be worried upon?How accurate is Hiv Ag/Ab CMIA test at 18 days? Today is 22nd day post our relationship and I still have a dry throat/phlegm stuck in throat, with no pain, no fever

Welcome back, but I don't understand why you thought it necessary. I reviewed your recent discussion with Dr. Hook and agree exactly with his evaluation and advice, and you have asked exactly the same questions again. That your penis might have touched your partner's vaginal opening means nothing in terms of HIV risk. Accordingly, my responses are brief.

"Given the latest understanding...are there any risks I need to be worried [about]?" No, none. Re-read your previous thread. You apparently assume or fear that HIV can be easily transmitted. Exactly wrong: even if you had had unprotected intercourse for 15 minutes, with no condom, and if your partner had HIV (which probably she did not), there would have been under one chance in a thousand of you becoming infected. The risk is truly zero with the kinds of events you have described. Believe it!

HIV Ag/Ab test:  About 80% of newlly infected persons would have positive results by that time. But you weren't at risk for HIV anyway. You don't need any more tests. However, if you insist, you can have a conclusive result 6 weeks after the event.

New HIV infection does not cause "throat/phlegm". This has nothing at all to do with the sexual non-exposure events you have described.

Don't confuse your anxieties about a sexual decision you regret with infection risk. They aren't the same. Just move on without further worry or testing. And please do not ask anything else that has already been answered.

My apologies. I did not mean to repeat my question. Pardon me for my average command of English. 

My actual question was:

- My friend whom I had exposure with on 27 May 2024 did a CMIA Ag/Ab Test on 14 June 2024 (18 Days post exposure). I did not do any testing. I actually wanted to ask if her test is a good indication for my assurance that she is indeed HIV free. I did a recap of my original question for convenience only.

- I understand that the penis touching opening of vagina is a no risk as shared by you and Dr. Hook. Truly anxiety kills. The reason I wanted clarity is I was reading about the risk if tip touch the clitoris, labia (inner/outer) or even the opening site of vagina where even a slight dipping can cause risk (as urethra can absorb vaginal fluid) freaked me out. To my knowledge and her knowledge, it was mere rubbing at the opening, but mind drives us all crazy as ordinary human. 

I once again apologize for the inconvenience.

There's no "inconvenience" to apologize for. The issue isn't inconvenience, it's your somewhat irrational fears. Yes, "anxiety kills", but we are not responsible for that. We can only provide HIV/STD related facts and our understanding of the science. The anxiety management part is up to you.

As I said above, a negative AgAb test at 18 days around 80% conclusive. Of course that's the same whether it's her test or your own. But she had little chance of having HIV anyway. You really are overreacting to a trivial risk situation!
