[Question #11494] Unprotected fellatio - Risk exposure

Hello doctor, I am man. I recently had unprotected fellatio with a woman for 10-15 seconds and protected vaginal sex for like 2 seconds and I freaked out and stopped. She said she is clean and  gets tested every 3 months and I have nothing to worry about. She had some sores on her legs but she assured that it was plaque psoriasis and She is clean. 

I tested for hiv, hepatitis, gonnorhea, clamidia and herpes on day 10 and everything was negative. Today is day 19 and I dont have any symptoms other than little urethra pain and I think it is because I keep checking for any sores or warths by pulling back my foreskin. I’m not circumcised aswell. I had throat swab yesterday for strep bacteria and now I feel like something is stuck in my throat. Doc said my throat is red, is that a symptom for sti? No leisons in throat. 

1. Is day 10 test conclusive for gonnorhea, clamidia which I might be risk of mostly? Do you know any case where day 10 test was negative for above and went on to become positive? 
2. Herpes - I dont have any gential warts till today, day 19. Ik you mentioned many times warts occur between 2-12 days at max.  Can I still get genital warts after 3 weeks which suggest herpes? If no warths does that mean I’m not infected by it? 
3. For syphilis, is there any max window to expect leisons to appear if infected? Is 28 day negative conclusive for syphilis? 
4. Hiv and hepatitis - Is 28 day negative result conclusive? 

I’m freaking out, please help.

I just got my results from mouth swab test. I tested positive for strep bacteria, other infections were negative. Doctor prescribed amoxicillin.

1. What are my risk exposures  to all the STI’s I mentioned above? (Gonnorhea, clamidiya, hiv, herpes and hepititis) 
 
2. If I take it for next week or 10 days, will that affect results for any sti tests? It says amoxicillin cures syphilis if I have it, so do you recommend any date for conclusive results where amoxicillin wont offset my results? 

FYI - Today is day 18. I’m paranoid and freaking out.  
I’m planning to give final test on day 28 for peace of mind.

I see you had another question recently that apparently you had deleted after we responded. Let's plan on this one remaining, OK? Presumably you have chosen a username took care in the question itself to assure no information that would betray your confidentiality. It's often helpful for the moderator to review prior discussions, especially if they concern the same exposure, symptoms, etc. Were these issues addressed in the deleted thread? If so, and if there is information not included in this question now, please let me know.

I'll start out by responding to the title you chose for this question:  even without protection, receiving fellatio is quite low risk for all STDs and zero risk for some, including HIV. It's good your partner believes she is "clean" and is tested frequently; these facts further lower the risk of any infection. Had you not already been tested, I would have advised it wasn't necessary. And for sure there was no point in testing for HIV, hepatitis, or herpes; and those tests were done too soon anyway. However, your urethra or urine test for gonorrhea/chlamydia was reliable.

As for your sore throat, no STD can cause that from the exposures you describe. By far the most likely explanation is a common viral infection, not an STD. While it is reasonable for your doctor to test for strep throat, I would expect a negative test result. All in all, you're overreacting a bit to an essentially zero risk situation. To your numbered questions:

1. The negative gon/chl test result is conclusive. It cannot become positive later.
2. You seem to be confused about two different STDs:  herpes and warts are entirely different. Herpes is caused by HSV and was almost no risk, and having no penile sores after 10 days is good evidence you were not infected. You do not need testing for it. Warts are caused by human papillomavirus (HPV) which is very rare from oral sex. Warts due to HPV don't show up sooner than 2-3 months after exposure. There is no test for HPV.
3. This was a very low risk exposure in regard to syphilis. The chancre -- i.e. the initial sore of syphilis -- typically appears 10-20 days after exposure. That's not going to happen.
4. You should not be tested for viral hepatitis; zero risk. HIV also was zero risk, but but because of its seriousness, testing is routine. At 28 days, a negative result will be 98-99% reliable. For a truly conclusive result, you'll need to be tested at 6 weeks.

"Freaking out" is not an appropriate response. You're at almost no risk. If somehow I were in your situation, I would not be getting tested for anything at all, and I would be continuing unprotected sex with my wife without worry. And in the 20 years of this and our preceding forum, with thousands of questions just like yours, not one person turned out to have actually acquired HIV or other serious STD. You won't be the first!

I'm glad your strep throat was diagnosed and you're on proper treatment for it.

1. Confusing question about risk. At the time of exposure, probably under one chance in a thousand for any of those infections. In view of your subsquent test results and lack of symptoms, under one in milions, as a rough guess.

2. Contrary to what you think you learned (online?), antibiotics generally do not simply interfere with accurate testing. They turn tests negative by curing the infection. With 10 days of amoxicillin, you will be cured of syphilis if you were exposed and you cannot possibly ever have a positive syphilis test. Don't waste your money on it.

Hello doctor, As I already mentioned I dont have any symptoms other than urethra irritation, which is mostly gone now. 
I haven’t had any sexual exposure with my wife since the risky exposure that I had outside which I mentioned you about. Its been 20 days now. We only kissed like few times and tongue was not invloved. She is having ingrown lumps under both her armpits today morning and its painful. 
I’m feeling guilty if I had anything and spread it to her? 

1. As I don’t have any symptoms, will I spread any sti to her just by kissing on lips(no tonuge), cheeks or using same towels, restrooms, water bottles? We dint have sex of kind oral or penetrative. 
I’m freaking out If I have herpes or HIV and gave I it to her that caused lumps under armpits. 
2. I’m not even sure If I have any STI. I gave another test today, day 20. Day 10 tests were negative. Please help.

No STI causes painful armpit swellings. If the bumps are in or on the skin -- i.e. red bumps you can see, maybe associated with hairs -- she has folliculitis or other superficial skin infection. If so, the strep causing your throat infection might also be infecting your wife's skin. She should see a doctor ASAP, the lesions tested for strep bacteria, and maybe treated with an antibiotic. However, these symptoms are not related to your sexual exposures with anyone else. I have given you strong reasons you can be sure you have no sexually transmitted infection. 

2. Maybe you are "not sure if [you] have any STI", but I am certain about it:  You do not.

There's nothing more I can help with. You and your wife should keep working with your doctor(s) about any continuing symptoms, especially to make sure your wife doesn't have a streptococcal skin infection. But please do your best to stop worrying about STIs.
