Hello doctors

Sorry for coming back with a new question and exposure, as I had a sexual encounter with a prostitute 23 days ago, as I was diagnosed with hsv2  , 4 months ago, and this encounter 3 weeks ago consisted of inserting my penis into her pussy for two seconds. I was wearing a condom and took it out immediately. Then I rubbed my penis on her anus while the condom was still there.

The rubbing was strong, which led to the condom breaking. I did not notice that. I continued to rub my penis on her anus for two minutes. It may have entered the head of my penis during this period. When she alerted me that the condom had broken, I got up. I stopped immediately and withdrew.

I have read a lot on this forum about such an event, but what is different here is that I was recently infected with HSV2, and this is considered a major risk factor, 3 times more likely for the transmission of HIV, hepatitis B, or syphilis. There are currently no symptoms, and it is the 23rd day since this incident, except that for two days I felt a desire to... Vomiting.

I would like you to kindly evaluate my situation and the possibility of my being infected with HIV, syphilis, or hepatitis B, knowing that I was recently infected with hsv2, which was confirmed by hsv2igg blood analysis 4 months after a suspicious relationship, and the result was a low positive of 1.01, and the normal range for the laboratory is less than 0.5, and the gray area 0.5 to 1, and the positivity is higher than 1, and my result is 1.01, close to the cut-off line. I have had continuous pain in the penis and red pimples in the pubic area for 4 months.

I missed to mention that this woman is the same woman accused of transmitting the HSV2 virus to me 4 months ago, which means that the risk is multiplied many times over now if we all have the HSV2 virus and her secretions were on my penis after the condom broke.

Welcome back to the Forum.  I will try to help but problems with translation as well as the fact that your testing appears to have been improper make it difficult to provide accurate answers.  I have reviewed your earlier interaction with me, as well as your previous two interactions with Ms. Warren and will summarize what I understand to be the case.

Following interactions with two commercial sex workers (CSWs) you developed a variety of symptoms including what you describe as "pills" (perhaps these are pimples or pustules?) at the base of your penis and perhaps on your penis as well.  Your wife has had similar sores on the lips of her vagina.  You've seen doctors and had blood tests which none of us here in the US are familiar with which have led to some confusion of whether or not you have herpes.  Blood tests for herpes are often unreliable.  From your history it appears that you get colds sores on your lips and if this is the case, you cannot get Herpes-1 on your genitals.  You had a biopsy of one of the genital lesions but the results did not show herpes and although additional special testing for herpes was recommended, it was not done.  I cannot tell whether or not you have herpes, or if you wife has herpes.  

FYI, herpes lesions rarely stay present for more than 10 days.  It sounds like your and/or your wife's lesions have been present for more than 10 days.  If that is the case, it is unlikely that the lesions (??pills?) you describe are herpes.

Now after all of this, apparently you returned to the CSW you had seen in the past a little over 3 weeks ago.  You wore a condom and it broke.  You may or may not have penetrated her.  You now want to know if you are at risk for syphilis, HIV or hepatitis B based on the situation you describe.  Again, I cannot tell if you have herpes on not.  If you do, it may increase your risk for HIV or hepatitis b but not syphilis slightly, perhaps as much as twice.  The best way to tell if you have these infections is to be seen by a KNOWLEDGEABLE health care provider and to get tested.  If the genital lesions you have described are still present, the best kind of doctor to see would be a dermatologist who can examine the lesions and do whatever testing is needed.  At this time, about 3 weeks after the event you describe, if you are infected, tests may be positive although test results for HIV, syphilis and hepatitis B will not be definitive until 6 weeks after your exposure.

I'm sorry I cannot help more.  I hope the summary and suggestions I have made have been helpful.  It is not clear to me that you have herpes on any other STI. EWH

Thank you, Dr. Hook. Everything you mentioned is true about my history and my problem now is that I entered into a sexual relationship with the same woman 3 weeks ago, and the relationship consisted of inserting my penis into her vagina while I was wearing a condom, for only two seconds, then I withdrew and rubbed my penis on her anus while the condom was still there. The strength of the rubbing, the condom broke, and I did not notice that. I continued rubbing my penis on her anus, and perhaps the head of my penis entered the anus when the condom broke. I continued rubbing while the condom was broken for two minutes, and when I noticed it was broken, I withdrew, and the relationship is now over, assuming that I am actually infected with HSV2, which she is. She is also infected with it and had the HIV virus and her secretions on my penis.

What are the chances of transmitting HIV and the hepatitis B virus to me in this case, knowing that there was no unprotected penetration, just rubbing on it Anus after the condom breaks.

Your follow is a bit repetitive. That said. If there was no unprotected penetration, there is no risk for HIV or hepatitis B, EW.H---

Well, Doctor, I know that this is my last interaction before closing this topic. I would like to confirm that rubbing my penis without protection on the anus without penetration, with her reproductive fluids on my penis, and in light of the fact that we are both infected with HSV2, does it not transmit HIV? 
Because I read through the forum threads that the target cells for HIV infection in an HSV2 patient are on the outer surface, and this makes the HIV transmission process easier.

I also forgot to mention: Should I be worried about any symptoms that will appear starting today, which is the 24th day since this incident, or is it too late for the symptoms to appear because they are outside the specific time during which they should appear?

You have asked the question of whether "rubbing my penis without protection on the anus without penetration, with her reproductive fluids on my penis, and in light of the fact that we are both infected with HSV2, does it not transmit HIV? "  before.  As before, the answer will NOT change.  There is no risk for HIV.

Once again, let me also state that it is NOT clear that you have HSV-2.

The symptoms of recently acquired HIV typically occur between 7 and 30 days after an exposure.  I doubt that your new symptoms are due to HIV however, if they were, a test would be positive at this time.
