Hello again and I hope you are well, dear doctors!
I'm back with some questions, clarifications:
1. 23 days after the last exposure, I did a rapid syphilis test (specificity over 98%) from the one that takes blood from the finger and it came out negative. I have no symptoms so far. and I was wondering what is the maximum period up to which syphilitic chancre can appear and if it only appears at the entrance gate, that is, when it comes into contact with the infected lesion, or maybe it also appears in another place?
2. What are the symptoms of ARS and what is the maximum period until which they can occur?
I haven't had any symptoms until now, just a little sore throat and chills, but probably from the cigarettes and the air conditioning.
I will do another laboratory test in 4 weeks for HIV and syphilis as Dr. Handsfield recommended for my peace of mind and I hope they will be negative.

Welcome back to the Forum although I must say, I'm sorry you felt the need following the reassurance provided by both Dr. Handsfield and I. We discourage repetitive questions and when clients continue to ask the same or similar questions repetitively we reserve the right to close the thread without a response and without return of your posting fee. BRief answers to your questions:
1. The average time for the onset of a syphilitic chancre is about 21 days. The appearance of chancres after 30 days is very uncommon. As previously point out to you, your doxycycline, combined with consistent condom use virtually guarantees that you did not acquire syphilis. You are worrying needlessly.
2. The symptoms of the classical ARS are typically severe, not mild and include high fever, severe sore throat, and widespread, severe joint and muscle aches. I'll remind you that Dr. Handsfield has already told you that your risk for HIV from the exposures you described was less than 1 in a million. If the symptoms you describe were due to HIV, an HIV test would be positive. Symptoms are caused by the presence of antibodies and the virus and for that reason, when symptoms are present, tests are always positive.
3. There are no good data on the conclusiveness of syphilis tests at 3 weeks. At 30 days the majority (>90% for sure) of persons who acquired syphilis would have positive tests.
Please no more repetitive questions about your virtually no risk encounters. EWH

If you are kind enough, please answer the question I asked before, namely what is the maximum period in which the symptoms of ARS can occur? I asked only once and did not receive an answer.
And I asked another question only once, still unanswered: where does syphilitic chancre appear? only in the place where the contact with the infected lesion of the partner took place or it can appear anywhere. For example, if the contact with the lesion occurred on the penis, can syphilitic chancre appear in the anus?
My personal opinion is that is good for other readers to know, in case they have such doubts! informative.

ARS symptoms do not occur after more than 30 days following an exposure.
Syphilis chancres are primary lesions which occur only at the point of contact with another lesion, not elsewhere.

Please tell me if Igm antibodies for syphilis appear first and how long after exposure do they appear? And how accurate is the testing of these antibodies 28 days (4 weeks) after exposure
Is this test relevant or should VDRL/RPR or other types of specific tests for treponemal antibodies be tested?
It is a question only to know in the future what and how to act in certain situations!

And the last thing, considering all the evaluations and advice received from you, a question that I have not addressed until now. Do you think it is useful to test myself for HIV and syphilis for 6 weeks? From a medical point of view, leaving aside my peace of mind???

IgM tests for syphilis are notoriously unreliable and are not recommended. RPR and VDRL tests detect both IgG and IgM antibodies to the syphilis bacteria. Treponemal tests become positive slightly (a few days) before the RPR or VDRL tests. Testing for syphilis using either RPR.VDRL tests of specific treponemal tests are equally accurate for syphilis diagnosis.
